PDCA循环管理在降低透析患者内瘘穿刺维护并发症中的应用

Application of PDCA circulation management in reducing complications of sputum puncture maintenance in dialysis patients

目的探讨PDCA循环管理在降低透析患者内瘘穿刺维护并发症中的应用价值。方法我院自2017年1月1日开始在内瘘血液透析患者中实施PDCA循环管理,以2017年1月~2017年10月作为本研究的观察组,取100例患者进行研究,同时以未实施PDCA循环管理的2016年全年为对照组,取100例患者进行研究,比较两组内瘘穿刺维护并发症的发生情况。结果对照组患者2016年全年共进行内瘘穿刺650例次,其中内瘘穿刺针脱针发生率为7.4%;观察组2017年1月~2017年10月共进行穿刺650例次,其中内瘘穿刺脱针发生率为4.0%;其中第一季度内瘘穿刺针脱针发生率为2.6%,第二季度则占1.4%;第三季度至2017年10月31日期间则无内瘘穿刺针脱针现象发生;观察组各阶段内瘘穿刺脱针率均明显低于对照组(P<0.05)。结论采用PDCA循环管理对行血液透析治疗的患者进行内瘘穿刺针管理可显著减少内瘘穿刺针脱针现象发生,值得在临床上推广应用。

Objective To explore the application value of PDCA circulation management in reducing the complications of sacral puncture maintenance in dialysis patients. Methods The PDCA cycle management was performed in patients with hemodialysis from January 1 st,2017.From January 2017 to October 2017,as the observation group of this study, 100 patients were studied.In 2016,the whole year of the PDCA cycle management was the control group,and 100 patients were studied to compare the occurrence of complications in the two groups.Results In the control group, 650 cases of internal hemorrhoid puncture were performed in 2016,among which the incidence of decompression of internal hemorrhoids was 7.4%. In the observation group,650 cases were puncture from January 2017 to October 2017.The incidence of puncture and depilation was 4.0%;in the first quarter, the incidence of puncture needle removal was 2.6%,and in the second quarter it was 1.4%;from the third quarter to October 31 st,2017,there was no internal puncture.The needle acupuncture phenomenon occurred;the rate of puncture and puncture in each stage of the observation group was significantly lower than that of the control group(P<0.05).Conclusion The use of PDCA circulation management for the treatment of hemodialysis patients with internal hemorrhoids can significantly reduce the occurrence of internal decubitus needle decoupling,which is worthy of clinical application.
