摘要
目的探讨术后延续健康指导对食管癌患者术后营养状况及生活质量的影响。方法选取2016年1~12月海南省人民医院收治的食管癌患者120例,按照随机数字表法随机分成对照组和观察组,每组60例。观察组患者术后给予延续健康指导;对照组患者术后给予传统的健康宣教,分别比较两组患者术后营养不良发生风险,不良情绪及生存质量的情况。结果干预后观察组患者的营养不良发生率为48.33%,低于对照组患者的发生率68.33%,两组发生率之间比较差异有统计学意义(χ~2=5.498,P<0.05);干预治疗后观察组的SAS、SDS得分较对照组患者低,差异有统计学意义(t=6.093、4.092,P<0.05);干预后观察组的各项生存质量评分均高于对照组,差异有统计学意义(t=6.337、3.641、3.919、7.168、4.825,P<0.01);观察组症状项目得分中除便秘、腹泻评分外,其余各项得分均低于对照组,差异有统计学意义(t=-2.751、-4.034、-2.461、-3.424、-2.594、-2.788,P<0.05)。结论术后延续健康指导能有减少食管癌患者营养不良的发生情况,改善患者的抑郁和焦虑等不良情绪,并有效提升患者的生活质量。
Objective To explore the impact of continuous health counseling on the quality of life of patients with esophageal cancer after the surgery. Methods 120 patients with esophageal cancer admitted to Hainan Provincial People′s Hospital from January 2016 to December 2016 were enrolled.According to the random number table method, they were randomly divided into control group(n=60) and observation group(n=60).After surgery, patients in the observation group were given continuous health counseling, while those in control group received traditional health service.The risk of postoperative malnutrition, unhealthy mood and the quality of life were compared between the two groups. Results After the intervention, the incidence of malnutrition in the observation group was 48.33%, which was lower than that in the control group(68.33%), the difference was statistically significant(χ~2=5.498, P<0.05).The scores of SAS and SDS in the observation group were significantly lower than those in the control group(t=6.093, 4.092, P<0.05).After the intervention, the scores of quality of life indices in the observation group were higher than that in the control group, the differences were statistically significant(t=6.337, 3.641, 3.919, 7.168, 4.825, P<0.01).Except for constipation and diarrhea scores, the scores of other clinical symptoms in the observation group were lower than those in the control group, and the differences were also statistically significant(t=-2.751,-4.034,-2.461,-3.424,-2.494,-2.594,-2.788, P<0.05). Conclusions Postoperative health counseling can reduce the incidence of malnutrition in patients with esophageal cancer, relieve the depression and anxiety of patients, and improve their quality of life.
