高龄二胎孕妇心电图及相关因素的分析

Changes of ECG, risk factors and effects on pregnancy in elderly pregnant women

目的探讨高龄孕妇(≥35岁)妊娠晚期心电图变化,危险因素及对妊娠的影响。方法选取2018年1月至2018年6月期间在我院产检并分娩的466例高龄孕妇作为观察组,另选取同期非高龄孕妇912例作为对照组,均在妊娠晚期接受ECG检查,对比分析危险因素及对妊娠的影响。结果观察组的窦性心动过速及过缓、房性早搏、室性早搏,室内传导阻滞,短PR间期、ST-T改变、低电压等发生率高于对照组(p<0.05)。观察组甲减、甲亢、妊娠高血压、妊娠糖尿病、子宫肌瘤、心肌病的发生率高于对照组(p<0.05)。观察组的剖宫产率和早产率均高于对照组,(p<0.05)。观察组中经产妇的剖宫产率和早产率均高于对照组,(p<0.05)。结论高龄二胎孕妇妊娠晚期更容易出现ECG异常特别是心律失常,对高龄的二胎妊娠妇女应该及时发现处理异常ECG,保护母婴安全,避免不良妊娠结局的发生。

Objective To investigate the changes of ecg during the third trimester of pregnancy, risk factors and the influence on pregnancy in elderly pregnant women(equal to or over 35 years old). Methods From January 2018 to June 2018,466 elderly pregnant women(equal to or over 35 years old) who admitted in Nanhai District People’s Hospital for prenatal examination and labor were selected in observation group, and during the same period, 912 pregnant women(less than 35 years old) were in control group. Women in both groups were examined by electrocardiogram during late stage pregnancy. To investigate the types and risk factors of cardiac arrhythmias and its influence during pregnancy. Results The rate of sinus tachycardia and bradycardia,room premature beat, ventricular premature beat, Indoor conduction block,short PR interphase, ST segment depression and low voltage of women in observation group were significantly higher than those of women in control group(p<0.05). The cesarean delivery rate and preterm birth rates in in observation group were significantly higher than those of women in control group(p<0.05).Conclusions The incidence of abnormal ECG of older pregnant women during second child pregnancy higher than those of pregnant women at the right age. Handle abnormal ECG in time to ensure the safety of mother and child, to avoid adverse pregnancy outcomes.
