超声及超声造影对卵巢良恶性肿瘤的诊断价值研究

Diagnostic value of ultrasound and contrast-enhanced ultrasonography in benign and malignant ovarian tumors

目的探讨超声及超声造影对卵巢良恶性肿瘤的诊断价值。方法选取58例卵巢恶性肿瘤患者和34例卵巢良性肿瘤患者。比较卵巢良恶性肿瘤的超声检查相关指标和超声造影特征,计算和分析超声及超声造影对卵巢恶性肿瘤的诊断价值,包括灵敏度、特异度、阳性预测值、阴性预测值和准确度。结果与卵巢良性肿瘤相比,卵巢恶性肿瘤的Alaczar超声评分和中心血流比例较高,阻力指数(RI)较低,超声造影开始增强时间、增强峰值时间和平均通过时间较短,增强峰值强度和增强曲线下面积较大(P﹤0.05)。超声及超声造影诊断卵巢恶性肿瘤的灵敏度、特异度、阳性预测值、阴性预测值和准确度分别为86.2%、88.2%、92.6%、78.9%和87.0%。结论超声及超声造影对卵巢恶性肿瘤的诊断价值较高,有助于对卵巢良恶性肿瘤进行鉴别诊断。

Objective To explore the diagnostic value of ultrasound and contrast-enhanced ultrasonography in benign and malignant ovarian tumors.Method Fifty-eight patients with malignant ovarian tumor and thirty-four patients with benign ovarian tumor were selected.The ultrasonic related indicators and contrast-enhanced ultrasonography features of benign and malignant ovarian tumors were compared,the diagnostic efficacy of ultrasound and contrast-enhanced ultrasonography in benign and malignant ovarian tumors was analyzed,including sensitivity,specificity,positive predictive value,negative predictive value,accuracy were calculated and analyzed.Result Compared with benign ovarian tumors,the Alaczar ultrasound score and the ratio of central blood flow were higher,the resistance index(RI)in malignant ovarian tumors was lower,initial enhancement time,peak enhancement time and mean transit time were shorter,peak enhancement intensity and area under enhancement curve were stronger and larger(P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of ultrasound and contrast-enhanced ultrasonography in detecting malignant ovarian tumors were 86.2%,88.2%,92.6%,78.9%and 87.0%respectively.Conclusion The diagnostic efficacy of ultrasound and contrast-enhanced ultrasonography in malignant ovarian tumors is high,which is helpful in the differential diagnosis of benign and malignant ovarian tumors.
