个性化管理对晚期肺癌化疗患者生活质量和心理状况的影响

Effect of individualized management on quality of life and psychological status of patients with advanced lung cancer undergoing chemotherapy

目的探讨个性化管理对晚期肺癌化疗患者生活质量和心理状况的影响。方法采用随机数字表法将136例晚期肺癌患者分为对照组和观察组,每组68例。对照组患者接受常规护理,观察组患者在此基础上接受个性化管理。比较护理前后两组患者的焦虑自评量表(SAS)、抑郁自评量表(SDS)、视觉模拟评分量表(VAS)、匹兹堡睡眠质量指数(PSQI)量表、生活质量评分情况,观察并比较护理2周后两组患者的不良反应发生情况。结果护理前,两组患者的SAS、SDS、VAS、PSQI、生活质量评分比较,差异均无统计学意义(P﹥0.05)。护理后,两组患者的SAS、SDS、VAS、PSQI评分均低于本组护理前,生活质量评分均高于本组护理前(P﹤0.05)。护理后,观察组患者的SAS、SDS、VAS评分均明显低于对照组患者,生活质量评分均明显高于对照组患者,差异均有统计学意义(P﹤0.01)。护理期间,两组患者均出现恶心呕吐、食欲减退、腹泻、失眠的不良反应。观察组患者的不良反应总发生率低于对照组患者(P﹤0.05)。结论个性化管理可明显改善晚期肺癌化疗患者的心理状况,减轻患者的疼痛,提高患者的睡眠和生活质量,降低不良反应总发生率,值得临床推广。

Objective To explore the effect of individualized management on the quality of life(QOL)and psychological status of patients with advanced lung cancer undergoing chemotherapy.Method A total of 136 patients with advanced lung cancer were included and randomized into two groups as control group and study group,with 68 cases in each,respectively.The control group was given routine nursing care,while the study group was administered with individualized management.Self-rating anxiety scale(SAS),self-rating depression scale(SDS),visual analogue scale(VAS),Pittsburgh sleep quality index(PSQI)scale and QOL scores were assessed before and after nursing care,the incidence of adverse reactions after 2 weeks of nursing was compared between the two groups.Result Before nursing,SAS,SDS,VAS,PSQI scale,and QOL scores were similar between the two groups(P>0.05),while all but QOL score,which was higher,were decreased after nursing care(P<0.05),besides,the SAS,SDS,and VAS scores were significantly lower in study group than in control group,and QOL score was higher instead,indicating statistically significant differences(P<0.01).During the nursing period,nausea and vomiting,anorexia,diarrhea,and insomnia were observed in both groups,while the overall incidence of adverse reactions in the study group was significantly lower than that in the control group(P<0.05).Conclusion Individualized management can significantly improve the psychological status of patients with advanced lung cancer undergoing chemotherapy,reduce the pain of patients,improve the quality of sleep and life of patients,reduce the total incidence of adverse reactions,worthy of clinical promotion.
