日间化疗中心PIVAS院购赫赛汀保存与使用管理及持续改进

Management of Herceptin and Continuous Improvement in PIVAS of Daytime Chemotherapy Center

目j的基于PDCA循环法对PIVAS院购赫赛汀的保存与使用进行有效的管理及持续改进,提升PIVAS药学服务模式,促进安全高效用药。方法纳入2015年9~12月期间使用赫赛汀的341例患者作为PDCA循环质量管理前的对照组,纳入2016年6月~2017年12月间使用赫赛汀的2219例患者作为质量管理后的观察组,同时纳入2018年1~12月间使用赫赛汀的3259例患者作为持续改进的观察组,对比使用PDCA循环法进行质量管理改进前后某院患者使用赫赛汀的情况。结果该院日间化疗中心PIVAS现行的院购赫赛汀管理模式规范了赫赛汀的应用流程,在对照组中使用赫赛汀的缺陷率为2.05%(7/341),在观察组中使用赫赛汀的缺陷率为0%(0/2219),有效杜绝了赫赛汀及其剩余药液结冰、超温、冻融等保存缺陷的发生。针对赫赛汀剩余药液过期的问题,采用赫赛汀按剂量收费模式,进一步优化了药物的保存和管理规程。结论日间化疗中心PIVAS基于PDCA循环法改进的院购赫赛汀管理实践规范了赫赛汀保存与使用的制度,保证了患者的临床安全合理用药。

OBJECTIVE To improve the management of preservation and use of Herceptin based on PDCA circulation,improve the pharmaceutical care model of PIVAS,and promote safe and efficient drug use.METHODS 341 patients who used Herceptin from September to December 2015 were included as the control group before PDCA cycle quality management,2219 patients who used Herceptin between June 2016 and December 2018 as the observation group after quality management.Meanwhile,3259 patients who used Herceptin from January to December 2018 were included as the continuous improvement observation group.The situation of patients using Herceptin before and after quality management improvement using PDCA cycle method was compared.RESULTS The current management mode of Herceptin in PIVAS daytime chemotherapy center standardized the configuration of Herceptin.The defect rate of using Herceptin in the control group was 2.05%(7/341),and that of using Herceptin in the observation group was 0%(0/2219),effectively preventing the occurrence of storage defects such as freezing,overheating,freezing and thawing of Herceptin and its residual liquid.Aiming at the problem of expiration of Herceptin residual solution,the dosage charging model of Herceptin was adopted to further optimize the preservation and management rules of Herceptin.CONCLUSION The system of Herceptin preservation and use is regulated according to practice of management on Herceptin purchased by PIVAS in daytime chemotherapy center based on PDCA circulation,ensuring the safety and rational use of drugs in patients.
