腹腔镜下卵巢囊肿剥除术治疗卵巢囊肿的临床效果

Clinical Effect of Laparoscopic Ovarian Cystectomy for Ovarian Cysts

目的探讨对卵巢囊肿患者选择腹腔镜下卵巢囊肿剥除术治疗后获得的临床效果。方法选取江苏省淮安市淮城卫生院妇产科收治的2013年5月至2018年5月收治的120例卵巢囊肿患者作为研究对象,按照抽签法随机分为观察组和对照组,每组60例。对照组接受开腹手术治疗,观察组接受腹腔镜下卵巢囊肿剥除术治疗。观察并比较两组相关手术指标(术中失血量、临床手术时间、首次进食时间、住院时间、术后下床活动时间),检测并比较两组激素水平[卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E2)],术后2 h和12 h,采用自制量表评估并比较两组疼痛程度。结果观察组术中失血量、临床手术时间、首次进食时间、住院时间、术后下床活动时间明显短于对照组,差异有统计学意义(P<0.05)。治疗前,两组FSH、LH和E2水平比较,差异无统计学意义(P>0.05);治疗后,观察组FSH和LH水平均低于同期对照组,E2水平高与同期对照组,差异均有统计学意义(P<0.05)。观察组术后2h疼痛评分以及术后12 h疼痛评分均明显低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜下卵巢囊肿剥除术治疗卵巢囊肿可明显降低术中失血量、临床手术时间、首次进食时间、住院时间、术后下床活动时间,改善激素水平,降低疼痛程度。

Objective To investigate the clinical effects of laparoscopic ovarian cystectomy in patients with ovarian cysts.Methods A total of 120 patients with ovarian cysts admitted to the Huai Town Health Center of Huai'an City,Jiangsu Province from May 2013 to May 2018 were selected as the study subjects.They were randomly divided into observation group and control group according to the sampling method,with 60 cases in each group.The control group received open surgery and the observation group received laparoscopic ovarian cystectomy.Observe and compare the relevant surgical indexes(intraoperative blood loss,clinical operation time,first feeding time,hospitalization time,postoperative outpatient activity time),and detect and compare the two groups of hormone levels[follicle stimulating hormone(FSH),corpus luteum formation Hormone(LH),estradiol(E2)],2 h and 12 h after surgery,the self-made scale was used to evaluate and compare the pain levels of the two groups.Results The blood loss,clinical operation time,first feeding time,hospitalization time,and time of getting out of bed in the observation group were significantly shorter than those in the control group,and the difference was statistically significant(P<0.05).Before treatment,the levels of FSH,LH and E2 in the two groups were not significantly different(P>0.05).After treatment,the FSH and LH levels in the observation group were lower than those in the control group,and the E2 level was higher than that in the control group.Statistically significant(P<0.05).The pain scores at 2 hours and the pain scores at 12 hours after operation were significantly lower in the observation group than in the control group(P<0.05).Conclusion Laparoscopic ovarian cystectomy for ovarian cysts can significantly reduce intraoperative blood loss,clinical operation time,first feeding time,hospital stay,postoperative time to get out of bed,improve hormone levels,and reduce pain.
