输尿管镜钬激光碎石术治疗输尿管结石的疗效观察

Clinical Observation of Ureteroscopic Holmium Laser Lithotripsy in the Treatment of Ureteral Calculi

目的观察输尿管镜下钬激光碎石术应用于输尿管结石中的疗效。方法选取2015年6月至2018年3月在德州市平原县第一人民医院接受治疗的76例输尿管结石患者,随机分为两组,每组38例,观察组患者接受输尿管镜钬激光碎石术治疗,对照组接受体外冲击波碎石术治疗。比较两组患者术后1个月结石排净率、手术时间和术后并发症发生率。结果观察组大于1 cm结石排净率高于对照组(P<0.05);两组小于1 cm结石排净率比较,差异无统计学意义(P>0.05)。观察组手术时间低于对照组,差异有统计学意义(P<0.05)。两组患者术后并发症发生率比较,差异无统计学意义(P>0.05)。结论在输尿管结石的临床治疗中,不同碎石手术均有各自优势,输尿管镜钬激光碎石术对于较大结石的碎石效果更为突出,可根据患者情况进行选用。

Objective To observe the effect of holmium laser lithotripsy under ureter microscope in the treatment of ureter calculi.Methods From June 2015 to March 2018,76 patients with ureter calculi were randomly divided into observation group(n=38)and control group(n=38).The observation group was treated with ureteroscopic holmium laser lithotripsy.The control group was treated with extracorporeal shock wave lithotripsy.The stone clearance rate 1 month post-operation,operation time,and postoperative complications of the two groups were compared.Results The clearance rate of stone more than 1 cm in the observation group was higher than that in the control group(P<0.05),while there was no significant difference in the clearance rate of stone less than 1 cm between the two groups(P>0.05).The operation time in the observation group were lower than that in the control group(P<0.05).There was no significant difference in the postoperative complications between the two groups(P>0.05).Conclusion Different lithotripsies had their own advantages in the clinical treatment of ureter calculi.Ureteroscopic holmium laser lithotripsy is more effective in the treatment of large stones and can be selected according to the situation of the patients.
