四肢创伤性骨折患者术后锻炼依从性与自我效能及应对方式的关系研究

Relationship between postoperative exercise compliance, self-efficacy and coping style in patients with traumatic fractures of extremities

目的探讨四肢创伤性骨折患者术后锻炼依从性与自我效能及应对方式的关系。方法选取2017年1月-2018年12月我院骨科收治的四肢创伤性骨折患者68例,采用骨科患者功能锻炼依从性量表、自我效能量表、应对方式量表对四肢创伤性骨折患者进行调查。结果四肢创伤性骨折患者术后锻炼依从性总评分为(32.5±3.8)分,单因素分析显示,不同年龄、学历、职业、医疗报销方式、疼痛评分患者锻炼依从性评分存在统计学差异(P<0.05)。Pearson分析显示,四肢创伤性骨折患者术后锻炼依从性评分与自我效能、积极应对方式评分呈正相关(P<0.05),而与消极应对方式评分呈负相关(P<0.05)。多元回归分析显示,年龄≥60岁、高中以下学历、消极应对方式是影响四肢创伤性骨折患者术后锻炼依从性危险因素,而积极应对方式是影响四肢创伤性骨折患者术后锻炼保护因素。结论通过提高四肢创伤性骨折患者自我效能将有助于患者以积极的态度面对疾病,从而提高患者锻炼依从性,有利于患者术后康复。

Objective To investigate the relationship between postoperative exercise compliance,self-efficacy and coping style in patients with traumatic fractures of extremities.Methods Totally 68 patients with traumatic fractures of the extremities admitted to our hospital in 2017 and 2018 were selected and investigated using the orthopaedic functional exercise compliance scale,self-efficacy energy scale,and coping style scale.Results The total exercise compliance score was(32.5±3.8),with significant differences among patients of different ages,education,occupation,medical reimbursement and pain scores(P<0.05).Pearson analysis showed that the postoperative exercise compliance score was positively correlated with the self-efficacy and positive coping styles(P<0.05),but negatively correlated with negative coping styles(P<0.05).The multiple regression analysis indicated that the age over 60,education lever below high school,and negative coping style were the risk factors for postoperative exercise compliance,while the positive coping style was the protective factor.Conclusion Improving the self-efficacy of patients with traumatic fractures of the extremities can help patients to face the disease with a positive attitude,improving their exercise compliance and facilitating postoperative rehabilitation.
