乳腺复杂硬化性病变与浸润性乳腺癌的MR影像比较

摘要 目的 比较乳腺复杂硬化性病变(CSL)和浸润性乳腺癌(IC)MR影像检查与病理分析特点。方法 选取2017年1月至2018年12月肇东市人民医院病理科收治的20例CSL患者与20例IC患者作为研究对象,回顾分析患者磁共振成像(MRI)检查与病理特点。结果 CSL患者年龄及病灶直径均小于IC患者,差异有统计学意义(P <0.05)。CSL与IC形态学及肿块边界比较,未见明显差异(P> 0.05)。IC患者T1WI序列病灶低信号多于CSL患者(P <0.05);两组病灶的T2WI与脂肪抑制(STIR)序列比较均未见明显差异(P> 0.05);动态增强扫描CSL与IC均表现均匀、不均匀强化;两组患者动态增强的时间-信号曲线(DCE-TIC)评估比较,差异有统计学意义(P <0.05);IC病变平均激痛点(ADC)值明显低于CSL患者,差异有统计学意义(P <0.05)。结论 IC与CSL相比在老年妇女群体多发,IC直径相对较大,且MR扫描中T1WI信号及ADC值更低,TIC曲线常见曲线类型为Ⅲ型。 Objective To compare the features of MR imaging and pathology of complex sclerosing lesions(CSL)and invasive carcinoma(IC)in the breast. Methods Twenty patients with CSL and 20 patients with IC who were admitted to the Department of Pathology of Zhaodong People’s Hospital from January 2017 to December 2018 were selected as the subjects. The magnetic resonance imaging(MRI)examination and pathological features were retrospectively analyzed. Results The age and lesion diameters of CSL patients were lower than those of IC patients(P<0.05).There was no significant difference in morphology and mass boundary between CSL and IC(P>0.05). The low signal in T1WI of IC patients was higher than that in the CSL patients(P<0.05). There was no significant difference in T2WI and STIR sequences between the two groups(P>0.05). The dynamic enhancement scans showed uniform and uneven intensification in both CSL and IC patients. The difference of DCE-TIC between the two groups was statistically significant(P<0.05). The mean ADC value of IC lesions was obviously lower than that of CSL patients(P<0.05).Conclusion Compared with CSL,IC is more common in older women,IC diameter is relatively larger,and T1WI signal and ADC value are lower in MR scan. The common curve type of TIC curve is type Ⅲ.
