精囊分泌功能与性功能关系的MR成像研究

Correlation research of seminal vesicle secretion function with sexual function using MRI

目的采用MR成像技术分析精囊分泌功能与性功能关系。方法选择2016年1月至2017年6月来恩施州中心医院生殖中心男科门就诊的54例性功能障碍患者为研究对象。按照患者主诉分类,主诉性欲淡漠为主25例患者设为A组;主诉勃起功能障碍(ED)为主29例患者设为B组;同时,另选30例性功能正常者设为C组。依据Raymond Rosen简化制定的勃起功能国际指数问卷表(IIEF-5,1998)对三组成员的勃起功能障碍程度进行评估,采用MR成像检测三组成员的精囊宽度,比较三组成员的精囊状况、精囊宽度、勃起功能障碍评分及性欲状况评分,并分析精囊宽度与性功能评分之间的相关性。结果精囊宽度与最长径比较结果显示:A组患者精囊宽度为(0.93±0.21)cm,B组患者为(0.86±0.18)cm,C组成员为(1.59±0.35)cm;A组患者精囊最长径为(1.27±0.22)mm,B组患者为(1.25±0.20)mm,C组成员为(1.54±0.07)mm;A组与C组成员、B组与C组成员之间精囊最长径与精囊宽度比较,其差异均有统计学意义(均P<0.05)。勃起功能障碍评分结果显示:A组患者评分为(15.93±3.31)分,B组患者为(12.84±3.52)分,C组成员为(23.84±4.62)分,A组与C组成员、B组与C组成员之间勃起功能障碍评分比较,其差异均有统计学意义(均P<0.05)。性欲状况评分结果显示:A组患者评分为(3.52±1.23)分,B组患者为(7.09±2.19)分,C组成员为(9.66±2.19)分,A组与C组成员、B组与C组成员之间性欲状况评分比较,其差异均有统计学意义(均P<0.05)。A组患者精囊宽度与性欲状况评分呈正相关(r=0.675,P<0.001),精囊宽度与勃起功能障碍评分呈正相关(r=0.824,P<0.001);B组患者精囊宽度与性欲状况评分呈正相关(r=0.668,P<0.001),精囊宽度与勃起功能障碍评分呈正相关(r=0.545,P<0.001);C组成员精囊宽度与性欲状况评分呈正相关(r=0.778,P=0.018),精囊宽度与勃起功能障碍评分呈正相关(r=0.609,P=0.009)。结论应用MR成像检查可以对精囊分泌功能进行评价,精囊分泌功能与性功能之间存在相关性。

Objective To analyze the correlation of seminal vesicle secretion function with sexual function using magnetic resonance imaging(MRI).Methods 54 patients with sexual dysfunction treated from January 2016 to June 2017 in the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture were selected and assigned to Group A(n=25,sex apathy),Group B(n=29,erectile dysfunction)and Group C(n=30,normal sexual function)according to the patient′s main complaint.Based on Raymond Rosen′s simplified International Index of Erectile Function 5(IIEF-5),the erectile dysfunction degree was evaluated.MRI was used to detect the width of seminal vesicle.The seminal vesicle condition and width,erectile dysfunction scores and sexual function scores were compared.The correlation of seminal vesicle width with sexual function was analyzed.Results The seminal vesicle width for Group A,Group B and Group C was(0.93±0.21)cm,(0.86±0.18)cm and(1.59±0.35)cm;the longest diameter of seminal vesicle for Group A,Group B and Group C was(1.27±0.22)mm,(1.25±0.20)mm and(1.54±0.07)mm;the width and the longest diameter of seminal vesicle between Group A and Group C,Group B and Group C was significantly different(all P<0.05).The erectile dysfunction scores for Group A,Group B and Group C were(15.93±3.31),(12.84±3.52)and(23.84±4.62)respectively,with statistically significant differences between Group A and Group C,Group B and Group C(all P<0.05);the evaluation scores of sexual function for Group A,Group B and Group C were(3.52±1.23),(7.09±2.19)and(9.66±2.19)accordingly,with statistically significant difference between Group A and Group C,Group B and Group C(all P<0.05).For Group A,the seminal vesicle width was positively correlated with sexual desire(r=0.675,P<0.001);the seminal vesicle width was positively correlated with the erectile dysfunction scores(r=0.824,P<0.001).For Group B,the seminal vesicle width was positively correlated with sexual desire(r=0.668,P<0.001);the seminal vesicle width was positively correlated with the erectile dysfunction scores(r=0.545,P<0.001).For Group C,the seminal vesicle width was positively correlated with sexual desire(r=0.778,P=0.018);the seminal vesicle width was positively correlated with the erectile dysfunction scores(r=0.609,P=0.009).Conclusions MRI can be used to evaluate the secretion function of seminal vesicle,which indicates that the secretion function of seminal vesicle is correlated with patients′sexual function.
