持续血液滤过治疗脓毒血症并急性肾衰竭的临床疗效

Clinical Effect of Continuous Hemofiltration in Treatment of Sepsis and Acute Renal Failure

目的探讨持续血液滤过治疗脓毒血症并急性肾衰竭的临床疗效,为临床治疗提供参考。方法选取云南省德宏州人民医院2017年9月至2019年1月收治的53例脓毒血症伴有急性肾衰竭患者作为研究对象,按照数字奇偶法随机分为观察组(26例)和对照组(27例)。对照组接受静脉血液透析滤过模式治疗,观察组接受持续静脉血液滤过模式治疗。观察并比较两组生命体征恢复正常情况、住院时间、连续性肾脏替代治疗时间、机械通气时间及不良反应发生情况。结果观察组患者生命体征恢复正常率(70.37%)高于对照组(42.31%),差异有统计学意义(P<0.05)。两组住院、连续性肾脏替代治疗及机械通气时间比较,差异均无统计学意义(P>0.05)。两组均未发生明显不良反应。结论脓毒血症伴有急性肾衰竭患者在接受连续性肾脏替代治疗期间,选择持续静脉血液滤过方式展开,对于生命体征恢复正常率的提升可获得显著效果。

Objective To investigate the clinical effect of continuous hemofiltration in the treatment of sepsis and acute renal failure.Methods A total of 53 patients with sepsis and acute renal failure admitted to the Dehong People's Hospital of Yunnan Province from September 2017 to January 2019 were randomly divided into observation group(26 cases)according to the numerical parity method.Control group(27 cases).The control group received venous hemodiafiltration mode and the observation group received continuous venous hemofiltration mode.The normal signs of hospitalization,hospitalization time,continuous renal replacement therapy time,mechanical ventilation time and adverse reactions were observed and compared.Results The normal rate of recovery of vital signs in the observation group(70.37%)was higher than that in the control group(42.31%),and the difference was statistically significant(P<0.05).There were no significant differences in hospitalization,continuous renal replacement therapy,and mechanical ventilation between the two groups(P>0.05).No significant adverse reactions occurred in either group.Conclusion Patients with sepsis associated with acute renal failure undergo continuous venous hemofiltration during continuous renal replacement therapy,which can achieve significant effects on the improvement of normal rate of vital signs.
