摘要
目的探讨冻干重组人脑利钠肽与左西孟旦联合应用治疗急性心力衰竭(AHF)的效果及安全性。方法随机选取南通大学附属医院2016年1月至2018年12月收治的AHF患者110例,分为研究组和对照组,每组55例。对照组在常规治疗的基础上加用左西孟旦,研究组在对照组的基础上加用冻干重组人脑利钠肽。比较两组临床疗效以及治疗前后左心室射血分数(LVEF)、左心室舒张末期内经(LVEDD)、每搏输出量(SV)、血清脑钠肽(BNP)水平,评价药物安全性。结果研究组的临床有效率明显高于对照组(94.55%vs.78.18%),差异有统计学意义(P<0.05);治疗前,两组患者LVEF、LVEDD、SV、BNP比较,差异无统计学意义(P>0.05);治疗后,研究组患者LVEF、SV明显高于对照组,LVEDD、BNP明显低于对照组,差异有统计学意义(P<0.05);两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论冻干重组人脑利钠肽与左西孟旦联合应用治疗AHF的疗效确切,可显著提高患者的心功能,明显降低血清BNP水平,药物安全性较高。
Objective To investigate the efficacy and safety of lyophilized recombinant human brain natriuretic peptide combined with levosimendan in the treatment of acute heart failure.Methods A total of 110 patients with acute heart failure admitted to our hospital from January 2016 to December 2018 were randomly divided into two groups:study group and control group,55 cases in each group.The control group was treated with levosimendan on the basis of routine treatment,while the study group was treated with lyophilized recombinant human brain natriuretic peptide on the basis of the control group.The clinical efficacy,left ventricular ejection fraction(LVEF),left ventricular end-diastolic menstruation(LVEDD),stroke output(SV)and serum brain natriuretic peptide(BNP)levels were compared between the two groups before and after treatment.Results The clinical effective rate of the study group was significantly higher than that of the control group(94.55%vs.78.18%,P<0.05).Before treatment,there was no significant difference in LVEF,LVEDD,SV and BNP between the two groups(P>0.05).After treatment,the LVEF and SV of the study group were significantly higher than those of the control group,and the LVEDD and BNP of the study group were significantly lower than those of the control group(P<0.05).There was no significant difference of the adverse events between the two groups(P>0.05).Conclusion The combination of lyophilized recombinant human brain natriuretic peptide and levosimendan is effective in the treatment of acute heart failure.The cardiac function of patients is significantly improved,the serum BNP level is significantly reduced,and the drug safety is high.
