不同剂量普瑞巴林用于单操作孔胸腔镜手术镇痛治疗的研究

Study on the Application of Different Doses of Pregabalin in the Analgesic Treatment of Uniport Thoracoscopic Surgery

目的胸腔镜术后疼痛的发生机制较为复杂,但有效的镇痛治疗是患者快速康复的重要前提。普瑞巴林是一种新型神经痛治疗药物,可用于胸部术后的镇痛治疗,但尚无明确的用药剂量标准。本研究目的是观察不同剂量的普瑞巴林在单操作孔胸腔镜术后的镇痛效果,探析其安全有效的使用方案。方法选取北京医院2015年10月至2017年1月收治的66例全麻单操作孔胸腔镜肺癌根治术的患者作为研究对象,将其随机分组为两组。大剂量组患者口服大剂量普瑞巴林,小剂量组口服小剂量普瑞巴林。术后随访入组患者,评估其急性和慢性疼痛的程度。结果两组患者的术后急性与慢性疼痛评分无统计学差异。大剂量普瑞巴林可显著增加嗜睡、头晕和视物模糊等不良反应。结论小剂量普瑞巴林能安全、有效地减少单操作孔胸腔镜患者的术后急性与慢性疼痛的发生。

Objective The mechanism of postoperative pain after thoracoscopic surgery is complex.However,effective analgesic treatment is very important for enhanced recovery after surgery(ERAS)of patients.As a new type of neuralgia drug,pregabalin can be applicated in the analgesic treatment of postoperative pain in thoracic surgery but without clear medication standard yet.In this study,pregabalin was applicated in uniport thoracoscopic surgery.The Results in postoperative analgesic treatment of different doses of pregabalin was to be observed,and the safe and effective application of pregabalin was to be explored.Methods A total of 66 patients of lung cancer,undergoing uniport thoracoscopic radical resection underwent general anesthesia from October 2015 to January 2017 were enrolled in this study.They were randomly divided into two groups.Patients in group A were given oral administration of high-dose pregabalin,and low-dose pregabalin for patients in group B.All patients were followed up to record the degree and the nature of postoperative acute pain and chronic pain.Results There was no statistically significant difference in postoperative acute and chronic pain scores between the two groups.In addition,high dose of pregabalin can increase the occurrence of somnolence,dizziness and blurred vision.Conclusion Low-dose administration of pregabalin in patients undergoing uniport thoracoscopic surgery can safely and effectively reduce the occurrence of postoperative acute pain and chronic pain.
