摘要 目的通过椎形束断层摄影系统(CBCT)图像研究下颌第一磨牙,为根管治疗提供可靠的理论基础。方法2017年7-9月于原成都军区口腔医学中心就诊的患者为研究对象,观察下颌第一磨牙CBCT图像,分析下颌第一磨牙近中中根管的起止部位及分布规律。结果181例326颗牙中有73颗(22.39%)存在近中中根管,20颗(6.13%)近颊与近舌根管之间存在峡部,8颗(2.45%)存在近颊或(和)近舌根管带尾。不同性别的患者下颌第一磨牙近中中根管比较,差异无统计学意义(P>0.05)。近中中根管在下颌第一磨牙中42.22%具有左右对称性,左右侧下颌第一磨牙近中中根管比较,差异无统计学意义(P>0.05)。结论下颌第一磨牙近中中根管的发生率较高,找到近中根额外存在的近中中根管并对其进行有效清理、充填,对根管治疗成功有重要意义。 Objective To study the mandibular first molars by the cone-beam computed tomographic(CBCT)images,in order to provide a reliable theoretical basis for the root canal therapy.Methods The patients treated in the original Chengdu Military Region Stomatology Medicine Center from July to September 2017 were served as the study subjects.The CBCT images of the mandibular first molars were observed.The position of beginning to end of the middle mesial canal of mandibular first molars and distribution rule were analyzed.Results Among 326 teeth in 181 cases,the middle mesial canal existed in 73 teeth(22.39%),the isthmus existed between the mesiobuccal and mesial sublingual root canal in 20 teeth(6.13%),and the mesiobuccal and/or mesiolingual root canal band tail existed in 8 teeth(2.45%).The appearance rate of the middle mesial canal had no statistical difference between different genders(P>0.05).Additionally,42.22%of the middle mesial canal of the mandibular first molars was left-right symmetric.The appearance rate of the middle mesial canal of mandibular first molars had no statistical difference betwwen the left and right sides(P>0.05).Conclusion The occurrence rate of middle mesial canal of the mandibular first molars is high.Finding out the additional middle mesial canal in mesial root,effectively clearing and filling these areas have important significance for the successful treatment of root canal.
