口腔恶性肿瘤患者术后溃疡出血的因素分析

Factors influencing postoperative ulcer bleeding in patients with malignant oral tumors

目的探讨口腔恶性肿瘤患者术后溃疡出血的因素。方法选取2014年3月至2018年3月间宝鸡市岐山县医院收治的83例口腔恶性肿瘤患者,均采用手术治疗,随访术后溃疡出血发生情况,分析影响因素。结果患者手术切口长度为(4.20±1.22)cm,手术时间为(165.92±22.28)min,术中出血量为(187.44±21.48)ml。术后随访1年,患者发生溃疡出血6例(7.2%)。单因素分析显示,患者术前白蛋白、血小板、血红蛋白、肿瘤直径、切口长度及手术时间,均与术后溃疡出血发生有相关性(均P<0.05)。以Logistic回归分析显示,血小板、白蛋白、血红蛋白及手术时间均为术后溃疡出血发生的主要因素(均P<0.05)。结论口腔恶性肿瘤患者术后溃疡出血较常见,血小板含量、白蛋白含量、血红蛋白含量及手术时间为溃疡出血的影响因素。

Objective To investigate the factors influencing postoperative ulcer bleeding in patients with malignant oral tumors.Methods From March 2014 to March 2018,83 patients with malignant oral tumors who were treated at Qishan County Hospital were selected as subjects.All patients were treated with surgery.The incidences of ulcer bleeding were investigated after the follow-up.Factors influencing the incidence ulcer bleeding were analyzed.Results The incision length was(4.20±1.22)cm.The operation time was(165.92±22.28)min.The blood loss was(187.44±21.48)ml during the operation.After 1 year of followed-up,ulcer bleeding occurred in 6 patients and the incidence rate were 7.2%.Univariate analysis showed that preoperative albumin,platelet,hemoglobin,tumor diameter,incision length and operation time were significantly correlated with postoperative ulcer bleeding(all P<0.05).Logistic regression analysis showed that platelet,albumin,hemoglobin and operation time were the main factors inflencing the incidence of postoperative ulcer bleeding(all P<0.05).Conclusion Postoperative ulcer bleeding is common in patients with malignant oral tumors.Platelet,albumin,and hemoglobin levels and operation time are factors influencing the occurrence of ulcer bleeding.
