四联疗法治疗急性期痔疮150例临床观察

Treatment of acute hemorrhoid by four combination therapy

目的寻求安全、简单、无痛、疗效好的治疗方法。方法选择2007年5月-2015年5月郑州大学医院收治的痔疮患者150例作为治疗组,采用术前中药熏洗加化痔栓塞肛,术中用微波组织间加热技术进行治疗,术后用TDP照射;另选50例痔疮患者作为对照组,采用常规手术治疗。所有痔疮患者均为急性炎症反应期。生化反应指标:肝肾功能及血糖水平正常,无严重心脏疾患,凝血三项正常,血清三项正常且无麻药过敏史。应用手术切除加(2450±50)MHz微波治疗仪微波去除,术后辅以中药熏洗和微波理疗,治疗各类急性期痔疮150例。结果 150例患者均为1次手术,痊愈147例,显效3例,未见复发。术时一般5~10 min,术后3天之内,大约有5例有排尿困难,经过下腹部揉按或者局部热水袋热敷20~30 min,3例可以排出,2例经过导尿处理。3例病人术后肛门皮肤水肿,经过10~15天局部TDP照射治疗,每天两次,每次20~30 min,水肿也随之慢慢消退。150例随访半年至3年,未见复发和后遗症等。结论采用手术加微波治疗和理疗及中药熏洗四联疗法与单纯手术比较,前者痊愈时间比后者快一倍,而且患者痛苦少,副反应少,并发症少,是值得推广和学习的治疗方法。该疗法治疗痔疮显效率达100%,该方法实用性强,有良好的应用前景。

Objective To seek a safe, simple, painless and effective treatment for acute hemorrhoid.Methods From May 2007 to May 2015, 150 patients with acne treated by our department were selected as the treatment group. Preoperative Chinese medicine fumigation and washing and phlegm embolization were used. The intraoperative technique was used for microwave interstitial heating. TDP irradiation;50 patients with acne were selected as the control group and treated with conventional surgery.All patients with acne are in an acute inflammatory response period.Biochemical reaction indicators: liver and kidney function and blood sugar levels were normal, no serious heart disease, three normal blood coagulation, three normal serum and no history of anesthesia.Surgical resection and(2450±50) MHz microwave therapy device were used for microwave removal. After operation, 150 cases of acute acne were treated with traditional Chinese medicine fumigation and microwave therapy.Results All 150 patients underwent one operation, 147 were cured, 3 were markedly effective, and no recurrence was found.The operation time is generally 5~10 minutes. Within 3 days after surgery, about 5 cases have difficulty in urinating. After the lower abdomen or local hot water bottle for 20~30 minutes, 3 cases can be discharged, and 2 cases are treated with catheterization.Three patients had postoperative anal skin edema. After 10~15 days of local TDP irradiation, twice a day for 20~30 minutes, the edema gradually subsided.150 patients were followed up for six months to three years, and no recurrence or sequelae were found.Conclusion Surgery plus microwave therapy and physiotherapy and traditional Chinese medicine fumigation and washing quadruple therapy compared with simple surgery, the former recovery time is twice as fast as the latter, and the patient has less pain, less side effects, less complications, it is worth promoting and learning. treatment method.The therapy has a visceral efficiency of 100%, and the method has strong practicability and good application prospects.
