乌司他丁在预防急性呼吸窘迫综合征发生中的作用研究

Effect of ulinastatin on prevention of acute respiratory distress syndrome

目的研究乌司他丁在预防急性呼吸窘迫综合征(ARDS)发生中的作用。方法回顾性分析晋城市人民医院2017年9月-2018年12月收治的44例有ARDS发生风险患者的临床资料,按照随机数字表法将其分为A组和B组,每组患者人数为22例。对A组患者给予临床常规治疗,B组患者在A组患者的治疗基础上加用乌司他丁。观察对比两组患者的临床治疗效果、治疗前后的肺功能指标、住院时间、入住ICU(重症监护病房)的时间以及病死率。结果对比两组患者治疗前的肺功能指标发现组间差异无统计学意义(P>0.05);对比两组患者治疗后的肺功能指标发现,B组各项指标显著优于A组,P<0.05,差异具有统计学意义;B组患者的住院时间、入住ICU的时间均短于A组,且病死率也低于A组,P<0.05,两组对比差异具有统计学意义;治疗后,B组ARDS发生率为9.09%(2/22),显著低于A组的31.82%,对比差异有统计学意义(P<0.05)。结论在治疗具有ARDS发生风险患者的过程中加用乌司他丁,疗效显著,能够有效改善患者的肺功能指标,且能够减少其住院时长,同时能够有效降低病死率,对预防ARDS有重要作用,值得推广使用。

Objective To study the effect of ulinastatin on the prevention of ARDS.Methods The clinical data of 44 patients at risk of ARDS admitted to our hospital from September 2017 to September 2018 were retrospectively analyzed. The patients were divided into group A and group B according to the random number table, and the number of patients in each group was 22. Patients in group A were treated with routine clinical treatment, and patients in group B were treated with ulinastatin on the basis of treatment in group A. The clinical treatment effect, pulmonary function indexes before and after treatment, length of stay, time of admission to ICU(intensive care unit) and case fatality of the two groups were compared.Results The lung function indexes of the two groups before treatment were compared. There was no statistically significant difference between the two groups. Comparing the pulmonary function indicators of the two groups after treatment, it was found that the indicators of group B were better than group A, withP<0.05, and the difference was statistically significant. The hospitalization time and ICU admission time of patients in group B were shorter than those in group A, and the case fatality rate was lower than that in group A,P<0.05. The difference between the two groups was statistically significant. After treatment, the incidence of ARDS in group B was 9.09%(2/22), significantly lower than 31.82% in group A.Conclusion The use of ulinastatin in the treatment of patients with risk of ARDS has significant therapeutic effect, which can effectively improve the indicators of lung function of patients, reduce the length of hospitalization, and effectively reduce the fatality rate. Therefore, it is of great importance to prevent ARDS and is worthy of promotion.
