子宫内膜异位症患者生育指数与腹腔镜术后自然妊娠的相关性

Correlation between fertility index and natural pregnancy after laparoscopic surgery in patients with endometriosis

目的探讨子宫内膜异位症患者生育指数(EFI)与腹腔镜术后自然妊娠的相关性。方法前瞻性分析2015年3月-2017年3月郑州大学第一附属医院收治的80例子宫内膜异位症患者,所有患者均接受腹腔镜手术治疗,同时对患者的一般资料(年龄、不孕年限、不孕类型等)进行收集,并记录术中相关情况,80例子宫内膜异位症患者均可进行有效随访,分析子宫内膜异位症生育指数(EFI)与术后妊娠的相关性。结果生育指数(EFI)评分在8、9、10分的子宫内膜异位症患者,其术后24个月累积妊娠率分别为77.78%(7/9)、81.82%(9/11)、87.50%(7/8),差异无统计学意义(χ~2=0.282,P=0.868);EFI评分在5、6、7分患者,术后妊娠率分别为70.00%(7/10)、66.67%(10/15)、68.14%(13/19),差异无统计学意义(χ~2=2.652,P=0.266);EFI评分在8~10分者与5~6分者术后24个月总妊娠率[82.14%(23/28)vs 68.18%(30/44)]比较,差异具有统计学意义(P<0.05)。结论生育指数(EFI)对子宫内膜异位症患者腹腔镜术后自然妊娠起到重要的指导作用。

Objective To investigate the correlation between endometriosis fertility index(EFI) and natural pregnancy after laparoscopic surgery in patients with endometriosis.Methods Totally 80 patients with endometriosis who admitted in the hospital from March 2015 to March 2017 were prospectively analyzed. All patients underwent laparoscopic surgery, and the general data of patients(age, infertility duration, infertility type, etc.) were collected, and the intraoperative situations were recorded. 80 patients with endometriosis were given effective follow-up. The correlation between EFI and postoperative pregnancy was analyzed.Results After surgery for 24 months, the cumulative pregnancy rates of patients with EFI score of 8, 9, 10 points were 77.78%(7/9), 81.82%(9/11), 87.50%(7/8), respectively, and the difference was not statistically significant(χ~2=0.282, P=0.868);The postoperative pregnancy rates of patients with EFI score of 5, 6, 7 points were 70.00%(7/10), 66.67%(10/15), 68.14%(13/19), respectively, and the difference was not statistically significant(χ~2=2.652, P=0.266);After surgery for 24 months, there was statistical difference in the total pregnancy rate between patients with EFI score of 8~10 points and patients with 5~6 points [82.14%(23/28) vs 68.18%(30/44)](P<0.05).Conclusion EFI plays an important role in guiding the natural pregnancy after laparoscopic surgery in patients with endometriosis.
