新型肝门血流阻断方法在腹腔镜下肝切除术中的应用效果

Application effect of new hepatic portal blood flow blocking method in laparoscopic hepatectomy

目的探讨新型肝门血流阻断方法在腹腔镜下肝切除中的应用效果。方法选取2018年1~12月在我院行腹腔镜肝切除手术的40例患者作为研究对象,按照肝门血流阻断方法的不同将其分为观察组(20例)和对照组(20例)。对照组患者采用传统肝门血流阻断方法治疗,观察组患者采取新型肝门血流阻断方法治疗。比较两组患者的肝门血流阻断时间、术中出血量、手术时间、术后住院时间及并发症总发生率。结果两组患者的肝门阻断时间、手术时间比较,差异无统计学意义(P>0.05);观察组患者的术中出血量少于对照组,观察组患者的术后住院时间短于对照组,差异均有统计学意义(P<0.05)。观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜下肝切除术新型肝门血流阻断方法能够有效减少术中出血量,缩短术后住院时间,值得临床推广应用。

Objective To explore the application effect of a new hepatic portal blood flow blocking method in laparoscopic hepatectomy.Methods A total of 40 patients who underwent laparoscopic hepatectomy in our hospital from January to December 2018 were selected as the research objects and divided into observation group(20 cases)and control group(20 cases)according to different methods of hepatic portal blood flow occlusion.Patients in the control group were treated with traditional hepatic portal blood flow blocking methods,while patients in the observation group were treated with new hepatic portal blood flow blocking methods.The portal blood flow blocking time,intraoperative blood loss,operation time,postoperative hospital stay and total incidence of complications were compared between the two groups.Results There were no significant difference in the time of hepatic portal occlusion and operation between the two groups(P>0.05).The intraoperative blood loss of the patients in the observation group was less than that of the control group,and the postoperative hospitalization time of the patients in the observation group was shorter than that of the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The new hepatic portal blood flow blocking method for laparoscopic hepatectomy can effectively reduce intraoperative blood loss and shorten postoperative hospital stay,which is worthy of clinical application.
