胆囊部分切除术治疗胆囊炎合并胆结石的临床效果

Clinical effect of partial cholecystectomy in the treatment of cholecystitis complicated with cholelithiasis

目的探讨胆囊炎合并胆结石患者采用胆囊部分切除术进行治疗的临床效果。方法选取2016年10月~2018年10月在我院接受手术的104例胆囊炎合并胆结石患者作为研究对象,通过随机数字表法将其分成对照组(52例)和治疗组(52例)。对照组患者采用常规胆囊全切除术进行治疗,治疗组患者采用胆囊部分切除术进行治疗。比较两组患者的治疗效果;比较两组患者的手术操作时间、术后引流时间、住院总时间;比较两组患者术后不同时间点的疼痛程度视觉模拟量表(VAS)评分。结果治疗组患者的手术治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗组患者的手术操作时间、术后引流时间、住院总时间短于对照组,差异有统计学意义(P<0.05);治疗组患者的术后并发症发生率低于对照组,差异有统计学意义(P<0.05);术后3 d,治疗组患者的疼痛程度VAS评分低于对照组,差异有统计学意义(P<0.05)。结论胆囊炎合并胆结石患者采用胆囊部分切除术进行治疗,可以减少术后并发症,缩短手术操作时间和术后引流及住院时间,有效减轻疼痛,降低术后并发症发生率,使手术治疗效果得到显著提升。

Objective To investigate the clinical effect of partial cholecystectomy in the treatment of cholecystitis complicated with cholelithiasis.Methods A total of 104 patients with cholecystitis complicated with cholelithiasis who underwent surgery in our hospital from October 2016 to October 2018 were selected as subjects.They were divided into control group(52 cases)and treatment group(52 cases)by random number table method.The control group was treated with conventional cholecystectomy.The treatment group was treated with partial cholecystectomy.The therapeutic effect of the two groups were compared.The operation time,postoperative drainage time and total hospital stay of the two groups were compared.The visual analogue scale(VAS)score of pain degree were compared between the two groups at different postoperative time points.Results The total effective rate of surgical treatment in the treatment group was higher than that in the control group,with statistically significant difference(P<0.05).The operation time,postoperative drainage time and total hospital stay of patients in the treatment group were shorter than those in the control group,with statistically significant differences(P<0.05).The incidence of postoperative complications in the treatment group was lower than that in the control group,with statistically significant difference(P<0.05).The VAS score of pain degree in the treatment group was lower than that in the control group 3 days after surgery,with statistically significant difference(P<0.05).Conclusion Partial cholecystectomy for cholecystitis complicated with cholelithiasis can reduce postoperative complications,shorten the operation time,postoperative drainage and hospitalization time,effectively relieve pain,reduce the incidence of postoperative complications,and significantly improve the surgical treatment effect.
