股骨近端抗旋髓内钉内固定术治疗股骨转子下骨折的临床效果

Clinical effect of proximal femoral anti-rotating intramedullary nailing in treatment of subtrochanteric fractures of the femur

目的探讨股骨近端抗旋髓内钉内固定术治疗股骨转子下骨折的临床效果。方法选取2016年12月~2018年12月我院收治的88例股骨转子下骨折患者作为研究对象,采用“单双数掷骰子”的方法将其随机分为对照组和观察组,每组各44例。对照组患者采用股骨近端锁定钢板内固定术,观察组患者采用股骨近端抗旋髓内钉内固定术,比较两组患者的围术期指标、手术治疗效果及术后并发症发生情况。结果观察组患者的手术时间、X线暴露时间及骨折愈合时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组患者的手术治疗优良率高于对照组,术后并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论对股骨转子下骨折患者应用股骨近端抗旋髓内钉内固定术的效果显著,可缩短手术时间,促进骨折愈合,值得推广。

Objective To investigate the clinical effect of proximal femoral anti-rotating intramedullary nailing in treatment of subtrochanteric fractures of the femur.Methods Eighty-eight patients with subtrochanteric fractures of the femur treated in our hospital from December 2016 to December 2018 were selected as the subjects.They were randomly divided into the control group and the observation group by the method of“single and double crows”,with 44 cases in each group.The patients in the control group were treated with proximal femoral locking plate fixation.The observation group was treated with proximal femoral anti-rotation intramedullary nailing.The perioperative indexes,surgical treatment effects and perioperative index were compared between the two groups.Results The operation time,X-ray exposure time and fracture healing time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).The excellent rate of surgical treatment in the observation group was higher than that in the control group,and the total incidence of postoperative complications was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of proximal femoral anti-rotation intramedullary nailing for patients with subtrochanteric fractures of the femur is effective,which can shorten the surgical treatment time and promote fracture healing.It is worthy of promotion.
