激痛点挑刺法对中风后肩痛及睡眠质量的影响

The Effect of MTRP on Shoulder Pain and Sleep Quality after Stroke

目的:观察激痛点挑刺法对中风后肩痛及睡眠质量的影响。方法:选取2017年6月至2018年9月福建中医药大学附属第三人民医院康复科收治的中风后肩痛患者60例作为研究对象,按照随机数字表法分为观察组和对照组,每组30例。2组均进行常规针刺及现代康复训练,观察组则在此基础上进行肩关节周围激痛点挑刺。观察2组患者治疗前后疼痛视觉模拟评分(VAS)、Constant-Murley肩关节功能评分、简化Fugl-Meyer上肢运动功能评分(FMA),匹兹堡睡眠评分(PSQI)的变化,并比较2组治疗的总有效率。结果:1)治疗后2组VAS评分、Constant-Murley肩关节功能评分、FMA评分,PSQI评分均较治疗前出现不同程度改善,差异均有统计学意义(P<0.05),其中观察组改善幅度优于对照组(P<0.05);2)观察组的总有效率为93.33%,与对照组的70.00%比较,差异有统计学意义。结论:激痛点挑刺法可明显缓解中风后肩痛的程度,由此改善患者的睡眠状况,临床可进一步推广应用。

Objective:To observe the effect of acupotomy on shoulder pain and sleep quality after stroke.Methods:A total of 60 patients with shoulder pain after stroke admitted to the rehabilitation department of the third affiliated people′s hospital of fujian university of traditional Chinese medicine from June 2017 to September 2018 were randomly divided into the observation group and the control group,with 30 patients in each group.Conventional acupuncture and modern rehabilitation training were carried out in both groups,while the observation group picked up the pain points around the shoulder joint on this basis.The changes of visual analogue pain score(VAS),constant-murley shoulder function score,simplified fugl-meyer upper limb motor function score(FMA)and Pittsburgh sleep score(PSQI)before and after treatment in the two groups were observed,and the total effective rate of the two groups was compared.Results:1)VAS score,constant-murley shoulder function score,FMA score and PSQI score in the two groups improved to different degrees after treatment,and the difference was statistically significant(P<0.05).The improvement in the observation group was better than that in the control group(P<0.05).2)the total effective rate of the observation group was 93.33%,which was statistically significant compared with 70.00%of the control group.Conclusion:MTRP can obviously relieve the degree of shoulder pain after stroke and improve the sleep status of patients.
