摘要
目的比较经玻璃体手术注射康柏西普或雷珠单抗治疗视网膜分支静脉阻塞(BRVO)继发黄斑水肿的临床疗效。方法回顾性分析2016年4月至2018年4月我院收治的经玻璃体手术注射康柏西普或雷珠单抗治疗的BRVO继发黄斑水肿110例(110患眼)患者的临床资料,均为单眼,根据注射的药物分为康柏西普组和雷珠单抗组,每组55例。比较2组患者治疗前及治疗后1、3、6个月最佳矫正视力(BCVA)与黄斑中心凹视网膜厚度(CMT),同时比较2组患者术中注射药物次数、治疗有效率、并发症发生情况。结果采用经玻璃体手术注射治疗BRVO继发黄斑水肿过程中,2组患者的BCVA均得到提高,但差异无统计学意义(P>0.05)。术后3、6个月康柏西普组的CMT改善效果优于雷珠单抗组,差异有统计学意义(t=2.35,P=0.021;t=2.60,P=0.011)。2组有效率均较高,且并发症较少,差异无统计学意义(P>0.05)。康柏西普组术中注射药物次数为(4.13±0.81)次,雷珠单抗组为(4.62±0.62)次,2组比较差异有统计学意义(P=0.0006)。结论经玻璃体手术注射康柏西普或雷珠单抗治疗BRVO继发黄斑水肿效果均较好,但康柏西普对于CMT改善效果更优、注射次数更少,而且价格相对便宜。
Objective To compare the treatment effect of vitreous injection of Conbercept or Lucentis on macular edema due to branch retinal vein occlusion(BRVO).Methods The clinical data of 110 patients of BRVO with macular edema(110 monocular eyes),who were treated with vitreous injection of Conbercept or Lucentis in our hospital from April 2016 to April 2018,were analyzed retrospectively.All the patients were divided into Conbercept group and Lucentis group,55 cases in each group.The BCVA and CMT values before treatment and after treatment of 1,3 and 6 months were compared.The injection times,effective rate of the treatment and complications rate were also compared.Results In the treatment of BRVO with macular edema by vitreous injection,both two groups got good results on BCVA improvement,with no significant difference(P>0.05).Conbercept group achieved better CMT improvement after 3 and 6 months than Lucentis group,the differences were significant(t=2.35,P=0.021;t=2.60,P=0.011).Both groups achieved high effective rates and low complication rate,with no significant difference(P>0.05).The injection times of Conbercept group was(4.13±0.81),that of Lucentis group was(4.62±0.62),there was significant difference between two groups(P=0.0006).Conclusion Vitreous injection of Conbercept and Lucentis both got good results in the treatment of macular edema due to BRVO.In consideration of better results in improving CMT,less injection times and the lower price,we recommend Conbercept as the optical choice in the treatment of macular edema due to BRVO.
