针刺联合悬吊训练治疗腰椎间盘突出症临床研究

Clinical Study on Effect of Acupuncture Combined with Sling Exercise Traing on Lumbar Disc Herniation

目的:观察针刺联合悬吊训练(SET)治疗腰椎间盘突出症(LDH)的临床疗效。方法:选取100例LDH患者,按随机数字表法分为对照组和观察组各50例。对照组给予尼美舒利胶囊口服,并对弱链肌群施行SET,观察组在对照组基础给予针刺治疗。2组均连续治疗3周。采用简化McGill疼痛问卷(SF-MPQ)评价腰痛程度,采用Oswestry功能障碍指数(ODI)和日本骨科协会(JOA)评分评价腰部运动功能,2组治疗前后各评价1次。结果:经秩和检验,观察组临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后,2组疼痛分级指数(PRI)、视觉模拟评分法(VAS)与现在疼痛状况(PPI)评分均较治疗前下降(P<0.01);观察组PRI、VAS与PPI评分均低于对照组(P<0.01)。治疗后,2组ODI评分均较治疗前下降(P<0.01),JOA腰痛主观症状、临床体征、日常工作能力评分和总分均较治疗前升高(P<0.01);观察组ODI评分低于对照组(P<0.01),JOA腰痛主观症状、临床体征和日常工作能力评分及总分均高于对照组(P<0.01)。结论:针刺联合SET治疗LDH,可以减轻患者的腰痛症状,改善腰部功能,临床疗效优于单纯使用SET治疗。

Objective:To observe the clinical effect of the therapy of acupuncture combined with sling exercise traing(SET)in the treatment of lumbar disc herniation(LDH).Methods:A total of 100 patients with LDH were selected and divided into the control group and the observation group according to random number table method,50 cases in each group.The control group was treated with nimesulli capsules orally and with SET for weak-link muscles;the observation group was additionally treated with acupuncture based on the treatment of the control group.Both groups were treated for three weeks.The degree of lower back pain was evaluated by the Short-Form McGill Pain Questionnaire(SF-MPQ),and the lumbar motor function was evaluated by the Oswestry Dysfunction Index(ODI) and the score of Japanese Orthopaedic Association(JOA).The two groups were respectively evaluated once before and after treatment.Results:The clinical effect in the observation group was better than that in the control group,the difference being significant(P<0.05).After treatment,the scores of Pain Rating Index(PRI),Visual Analogue Scale(VAS) and Present Pain Index(PPI) in the two groups were all decreased when compared with those before treatment(P<0.01).The scores of PRI,VAS and PPI in the observation group were lower than those in the control group(P<0.01).After treatment,the scores of ODI in both groups were decreased when compared with those before treatment(P<0.01),and the scores of subjective symptoms,clinical body signs and daily work ability in JOA and the total scores were increased when compared with those before treatment(P<0.01).The ODI score in the observation group was lower than that in the control group(P<0.01),and the scores of subjective symptoms,clinical body signs and daily work ability in JOA and the total score were higher than those in the control group(P<0.01).Conclusion:The therapy of acupuncture combined with SET for patients with LDH can relieve their lower back pain and improve the lumbar function,whose clinical effect is superior to that of SET therapy alone.
