温针灸联合独活寄生汤治疗肝肾亏虚型膝关节骨性关节炎临床研究

Clinical Study on Warming Needle and Moxibustion Combined with Duhuo Jisheng Tang for Knee Osteoarthritis of Liver and Kidney Deficiency Type

目的:观察温针灸联合独活寄生汤治疗肝肾亏虚型膝关节骨性关节炎(KOA)的临床疗效。方法:选取64例肝肾亏虚型KOA患者为研究对象,随机分为观察组与对照组各32例。观察组给予温针灸联合独活寄生汤治疗,对照组给予口服塞来昔布胶囊治疗。比较2组临床疗效,观察2组治疗前后白细胞介素-1 (IL-1)、基质金属蛋白酶-3 (MMP-3)水平的变化,采用WOMAC骨关节炎指数评价患者的膝关节功能,记录不良反应发生情况。结果:总有效率观察组93.75%,对照组75.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组IL-1、MMP-3水平均较治疗前降低(P<0.05);观察组IL-1、MMP-3水平均低于对照组(P<0.05)。治疗后,2组关节疼痛、僵直、功能障碍评分均较治疗前降低(P<0.05);观察组关节疼痛、僵直、功能障碍评分均低于对照组(P<0.05)。结论:温针灸联合独活寄生汤治疗肝肾亏虚型KOA疗效显著,可有效缓解疼痛,改善关节功能,减轻炎症反应,值得推广应用。

Objective:To observe the clinical effect of warming needle and moxibustion combined with Duhuo Jisheng tang for knee osteoarthritis(KOA) of liver and kidney deficiency type.Methods:Selected 64 cases of KOA patients of liver and kidney deficiency type as research objects and divided them into the observation group and the control group randomly,with32 cases in each group.The observation group was treated with warming needle and moxibustion combined with Duhuo Jisheng tang,while the control group was given the oral application of celecoxib capsules.Compared the clinical effect in the two groups and observed the level changes of interleukin-1(IL-1) and matrix metalloproteinase-3(MMP-3) in the two groups before and after treatment.Evaluated the knee joint function of the patients by applying WOMAC osteoarthritis indexes,and recorded the occurrence of adverse reactions.Results:The total effective rate was 93.75% in the observation group and75.00% in the control group,compared the two groups,difference being significant(P<0.05).After treatment,the levels of IL-1 and MMP-3 in the two groups were decreased when compared with those before treatment(P<0.05);the levels of IL-1 and MMP-3 in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of arthralgia,rigor and dysfunction in the two groups were all decreased when compared with those before treatment(P<0.05).The scores of arthralgia,rigor and dysfunction in the observation group were lower than those in the control group(P<0.05).Conclusion:The application of warming needle and moxibustion combined with Duhuo Jisheng tang for KOA of liver and kidney deficiency type has obvious curative effect,which can effectively relieve the pain,improve the joint function and reduce the inflammatory response.It is worthy of promotion and application.
