系统评价肺康复护理对慢性阻塞性肺疾病患者肺功能的影响

Systematic Review on the Effect of Pulmonary Rehabilitation Nursing on Lung Function in Patients with Chronic Obstructive Pulmonary Disease

目的:系统评价肺康复护理对慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者肺功能和生活质量的影响。方法:检索中国知网、Pubmed、Cochrane、Embase等数据库中关于肺康复护理对COPD患者肺功能和生活质量影响的随机对照试验,同时筛查纳入文献的参考文献。由3名研究者一起对文献质量进行严格筛选评价和资料提取,对符合质量标准的研究文献进行Meta分析。结果:与常规护理比较,肺康复护理能够升高COPD病人6 min步行距离运动耐量,改善生活质量,合并效应均有统计学意义(Z=4.08,P<0.000 1)。但对COPD患者的第1 s用力呼气量、用力肺活量、第1 s用力呼气量/用力肺活量干预效果不明显。结论:肺康复护理干预在提高COPD患者的6 min步行距离运动耐量以及改善生活质量方面显示出积极的效果。

Objective:To systematically review the effect of pulmonary rehabilitation nursing on lung function and quality of life in patients with chronic obstructive pulmonary disease(COPD).Methods:Searched the databases such as China national knowledge infrastructure,Pubmed,Cochrane and Embase for randomized controlled trials of the effects of pulmonary rehabilitation nursing on lung function and quality of life in COPD patients and screened the references included in the literature.Three researchers strictly conducted a rigorous screening evaluation and data extraction on the quality of the literature,and gave a meta-analysis of the research literature which met the quality standard.Results:Compared with routine nursing,pulmonary rehabilitation nursing increased the exercise tolerance of walk distance within 6 min in COPD patients and improved the quality of life;the merged effect had significant differences(Z=4.08,P<0.000 1).However,the intervention effect was not obvious on forced expiratory volume in first second,forced vital capacity and forced expiratory volume in first second/forced vital capacity in COPD patients.Conclusion:The intervention of pulmonary rehabilitation nursing shows positive effect on enhancing the exercise tolerance of walk distance within 6 min in COPD patients and on improving the quality of life.
