血小板参数与类风湿关节炎临床指标相关性研究

A correlation study between platelet parameters and rheumatoid arthritis

目的:探讨血小板参数与类风湿关节炎之间的关系。方法:采用回顾性分析方法选取2016年2月至2019年2月本院就诊的45例类风湿关节炎(RA)病人,另外选择60例健康体检志愿者。通过t-test比较血小板参数在两组人群间差异;结合RA相关临床指标,利用斯皮尔曼(Spearman)相关性分析,研究血小板参数与RA间的相关性。结果:与健康对照比较,血小板计数(PLT:)显著升高(P<0.05),而平均血小板体积(MPV)和血小板分布宽度(PDW)在RA患者中下调(f><0.05)。PLT与类风湿因子(R=0.437,P=0.007),血沉(R=0.371,P=0.016)和C反应蛋白(R=0.530,P<0.001)存在显著正相关。MPV与血沉(R=-0.403,P=0.012)和C反应蛋白(R=-0.375,P=0.022)负相关。PDW与血沉(R=-0.447,P=0.005)和C反应蛋白(R=-0.509,P=0.001)负相关。抗环瓜安酸肽抗体与血小板各参数间皆不存在相关性。结论:RA患者血小板参数出现异常,并与RA相关的临床特征存在显著相关,对提高RA临床诊断具有重要意义。

Objective:To investigate the correlation of platelet parameters and rheumatoid arthritis(RA).Methods:RA patients(N=45)and healthy controls(N=60)were collected and analyzed from February 2016 to February 2019 by using a retrospective study.The difference of platelet parameters were compared between two groups with t-test The correlation of platelet parameters and RA were investigated with Spearman correlation study.Results:Compared with health controls,PLT was higher,while MPV and PDW were lower in RA patients.PLT was positively correlated with RF(R=0.437,P=0.007),ESR(R=0.371,P=0.016)and CR P(R=0.530,P<0.001).However,MPV was negatively correlated with ESR(R=-0.403,P=0.012)and CRP(R=—0.375,P=0.022).Similarly,PDW was also correlated with ESR(R=-0.447,P=0.005)and CRP(R=—0.509,P=0.001).CCP has no relationship with platelet parameters.Conclusion:Platelet parameters were different in RA patients compared with the healthy control,and correlated with RA clinic characteristics.Thisresult will provide insight into improving RA clinic diagnosis.
