摘要
目的探讨帕金森病(PD)病人认知水平和肾小球滤过率(GFR)的关系。方法选取常熟市中医院94例原发性PD病人,对所有病人使用蒙特利尔认知量表(MoCA)、统一的帕金森病评估量表第三部分(UPDRS-Ⅲ)(开期)、Hoehn&Yahr分期进行评定;并计算左旋多巴等效剂量(levodopa equivalent dose,LED)。通过Cockcroft-Gault公式计算估算的GFR(eGFR),并与其他临床资料进行相关性分析。结果所有参与研究的94例PD病人平均年龄为(63.28±9.46)岁,其中男64例(68.1%),平均受教育年限为(9.27±4.57)年,经修正后的MoCA平均得分为(22.49±5.02)分。病人MoCA评分与UPDRS-Ⅲ评分(r=-0.109,P=0.015)、Hoehn&Yahr分期(r=-2.177,P=0.006)呈负相关;与教育水平(r=0.354,P=0.001)、eGFR(r=0.042,P=0.043)呈正相关;线性回归模型可以预测约28.5%的MoCA得分变异。结论GFR、教育水平、疾病严重程度都与PD病人的认知水平相关。
Objective To explore the relationship between cognitive function and glomerular filtration rate(GFR)in the patients with Parkinson's disease(PD).Methods A total of 94 PD patients admitted in Changshu TCM Hospital Affiliated to Nanjing University of Chinese Medicine from January 2008 to June 2019 were enrolled consecutively.All patients were assessed by using Montreal Cognitive Assessment(MoCA),Unified Parkinson Disease Rating Scale PartⅢ(UPDRS-Ⅲ)and Hoehn and Yahr stage.Levodopa equivalent dose(LED)was calculated.Estimated glomerular filtration rate(eGFR)was calculated by Cockcroft-Gault formula.Results Of all 94 PD patients,the average age was 63.28±9.46 years old,with 64 cases of male(68.1%).The average education year was 9.27±4.57 years,and the modified MoCA score was 22.49±5.02.The modified MoCA score was significantly negatively correlated with UPDRS-Ⅲ score(r=-0.109,P=0.015)and Hoehn and Yahr stage(r=-2.177,P=0.006),and was significantly positively correlated with education years(r=0.354,P=0.001)and eGFR(r=0.042,P=0.043).The linear regression model fitted the original data well and could explain 28.5% of the MoCA score variation.Conclusions GFR,education years,severity of the disease are correlated with the cognitive function in PD patients.
