补肾清毒化瘀煎肾区离子透入治疗慢性肾衰竭疗效观察

Observation on treatment of chronic renal failure with Bushen Qingdu Huayu Decoction by iontophoresis at renal area

目的观察补肾清毒化瘀煎肾区离子透入治疗慢性肾衰竭(CRF)的临床疗效。方法将60例CRF患者随机分为观察组30例和对照组30例,对照组给予常规治疗,观察组在其基础上给予补肾清毒化瘀煎肾区离子透入治疗,以10 d为1个疗程,连续治疗3个疗程。比较2组临床疗效,观察2组治疗前后中医证候积分、肾功能、电解质和营养学指标的变化。结果与治疗前比较,2组治疗后腰膝酸软、倦怠乏力、食少纳呆、头晕、脘腹胀满、恶心呕吐、口干口苦、大便干结、尿少色黄积分和总分及血肌酐(Cr)、血尿素氮(BUN)、24 h尿蛋白、每高倍镜视野红细胞数、每低倍镜视野蜡状管型数及血磷、钙磷乘积、血钾水平均显著降低(P均<0.05),内生肌酐清除率(Ccr)、血钙、血钠、血镁、二氧化碳结合力、红细胞(RBC)计数、血红蛋白(Hb)、血清总蛋白(TB)、血清白蛋白(ALB)水平均显著升高(P均<0.05);与对照组治疗后比较,观察组以上指标改善情况均优于对照组(P均<0.05)。观察组中医证候和临床疾病治疗有效率均显著高于对照组(P均<0.05)。结论补肾清毒化瘀煎肾区离子透入治疗CRF不仅能显著缓解临床症状,减轻肾功能恶化,还能改善电解质紊乱,纠正代谢性酸中毒和贫血,并改善机体营养状态。

Objective It is to observe the clinical efficacy of Bushen Qingdu Huayu Decoction by iontophoresis at renal area on chronic renal failure(CRF).Methods Sixty patients with CRF were randomly divided into the observation group(n=30)and the control group(n=30).The control group was treated with routine treatment.The observation group was treated with Bushen Qingdu Huayu Decoction by iontophoresis at renal area based on the control group.10 days as one treatment course,and both groups were treated for 3 courses.The clinical efficacy of the two groups was compared,and the changes of TCM syndrome scores,indicators of renal function,electrolytes and nutritional before and after treatment were observed.Results Compared with before treatment,the scores of weak waist and knees,fatigue,poor appetite,dizziness,abdominal distension,nausea and vomiting,dry mouth and bitter taste,dry stool,less and yellow urine and total scores and levels of serum creatinine(SCr),blood urea nitrogen(BUN),24h urine protein,number of red blood cells per high power microscope,number of waxy tubes per low power field,blood phosphorus,calcium and phosphorus product,blood potassium were significantly decreased(P<0.05),endogenous intoxication clearance(Ccr),serum calcium,blood sodium,blood magnesium,carbon dioxide binding,red blood cell(RBC)count,hemoglobin(Hb),serum total protein(TB),serum albumin(ALB)were significantly increased in the two groups(P<0.05),the improvement of the above indicators in the observation group was better than that of the control group(P<0.05).The effective rate of TCM symptom and clinical disease treatment in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Bushen Qingdu Huayu Decoction by iontophoresis at renal area can not only relieve clinical symptoms,reduce renal function deterioration,but also improve electrolyte imbalance,correct metabolic acidosis and anemia,and improve the nutritional status of the body in the treatment of CRF.
