易误诊的嗜酸粒细胞胃肠炎1例分析

Analysis of one case of misdiagnosed eosinophilic gastroenteritis

嗜酸粒细胞胃肠炎(EG)是临床少见的肠道炎症疾病,因其临床表现缺乏特异性,故该病的误诊率极高。本文选取2017年北京中医药大学东方医院(以下简称“我院”)肝胆脾胃科门诊接诊的1例EG进行报道,该患者因表现为腹痛伴脓血便,结合结肠镜下水肿、充血、溃疡等表现,早期考虑为溃疡性结肠炎,但治疗效果不佳,于我院复查肠镜并行病理检查,重新诊断为EG,更改相应治疗方案后,患者症状得以缓解。为提高EG认识,回顾相关文献并分析两种疾病的鉴别点,其中结肠镜及病理检查在两种疾病的诊断及鉴别上尤为重要。

Eosinophilic gastroenteritis(EG)is a rare intestinal inflammatory disease in clinic.Because of its lack of specificity in clinical manifestations,the misdiagnosis rate of EG is very high.This paper reports a case of EG in Outpatient Department of Hepatobiliary and Gastroenterology of Dongfang Hospital in Beijing University of Traditional Chinese Medicine(hereinafter referred to as"our hospital")in 2017.Because of abdominal pain and purulent stool,combined with colonoscopic edema,congestion and ulcer,the patient is considered as ulcerative colitis in the early stage.However,the therapeutic effect is not good.The patient′s symptoms are alleviated after re-examination of colonoscopy and pathological examination in our hospital and re-diagnosis of EG.In order to improve the understanding of EG,the relevant literature is reviewed and the differential points between two diseases are analyzed.Colonoscopy and pathological examination are particularly important in the diagnosis and differential diagnosis of the two diseases.
