腹腔镜经自然腔道取标本手术治疗乙状结肠/直肠癌患者的近期疗效及安全性

Short-Term Efficacy and Safety of Natural Orifice Specimen Extraction Surgery in the Treatment of Patients with Colorectal Cancer

目的分析乙状结肠/直肠癌腹腔镜采用经自然腔道取标本手术(NOSES)治疗的近期治疗疗效以及安全性。方法选取2016年6月至2019年3月我院肿瘤外科收治的80例乙状结肠/直肠癌患者作为研究对象,根据患者手术方案进行分组,其中采用NOSES手术方案进行治疗的42例患者为观察组,采用普通腹腔镜手术进行治疗的38例患者作为对照组,比较2组患者手术康复指标、炎症指标、疼痛评分以及并发症发生情况。结果观察组患者术后首次排气时间、首次排便时间以及住院时间明显短于对照组患者,差异均有统计学意义(P均<0.05)。2组患者术前C-反应蛋白(CRP)表达水平比较差异无统计学意义(P>0.05)。观察组患者术后第1、3、5天CRP表达水平均明显低于对照组,差异均有统计学意义(P均<0.05)。观察组患者术后第1、3、5天疼痛评分均明显低于对照组,差异均有统计学意义(P均<0.05)。2组患者术后并发症总发生率比较差异统计学意义(P>0.05)。结论乙状结肠/直肠癌患者采用NOSES手术的近期疗效显著,且安全可靠。

Objective To analyze the short-term efficacy and safety of natural orifce specimen extraction surgery(NOSES)in the treatment of patients with colorectal cancer.Methods From June 2016 to March 2019,80 patients with colorectal cancer in the Department of Oncological Surgery of Xinyang Central Hospital were enrolled as the subjects.According to the surgical plan,42 patients with NOSES plan were used as the observation group,and 38 patients with common laparoscopic surgery were used as the control group.The surgical rehabilitation index,inflammation index,pain score and complications were compared between the two groups.Results The first exhaust time,first defecation time and hospitalization time of the observation group were significantly shorter than those of the control group(P<0.05).There was no significant difference in the preoperative C-reactive protein(CRP)expression between the two groups(P>0.05).The expression of CRP on the first,third and fifth day after operation in the observation group were significantly lower than those in the control group(P<0.05).The pain scores of the observation group on the first,third and fifth day after operation were significantly lower than those of the control group(P<0.05).There was no significant difference in the total postoperative complication incidence between the two groups(P>0.05).Conclusion NOSES is effective and safe in the treatment of patients with colorectal cancer.
