快速康复外科理念在开胸肺切除术围术期护理中的应用

Application of rapid rehabilitation surgery concept in perioperative nursing of thoracotomy pneumonectomy

目的探讨快速康复外科理念在开胸肺切除术围术期护理中的应用效果。方法选取我院2017年2月至2018年2月收治的76例择期开胸肺切除术患者为研究对象,将其随机等分为研究组和对照组,对照组实施常规护理,研究组实施基于快速康复外科理念的护理,比较两组术中出血量、应激水平指标、并发症及住院时间。结果两组术中出血量比较差异无统计学意义(P> 0. 05);研究组并发症发生率低于对照组(P <0. 05),住院时间短于对照组(P <0. 05);研究组术后24 h与72 h外周血白细胞计数(WBC)、细胞因子白介素-6(IL-6)、C反应蛋白(CRP)水平均明显低于对照组(P <0. 05)。结论应用快速康复外科理念不增加开胸肺切除术中出血量,可有效减少术后并发症,降低各项应激反应指标,提高恢复效率和效果,缩短住院时间,促进机体快速康复。

Objective To explore the application effect of rapid rehabilitation surgery concept in perioperative nursing of thoracotomy pneumonectomy.Methods To select 76 cases of patients undergoing elective thoracotomy pneumonectomy who were admitted to our hospital from February 2017 to February2018 as a study object. They were randomly divided into study group and control group,the control group was given routine nursing,and the nursing based on rapid rehabilitation surgery concept was implemented in the study group. Comparing the intraoperative blood loss,stress level indicators,complications and hospitalization time. Results There was no significant difference in intraoperative blood loss between the two groups( P > 0. 05). The incidence of complications in the study group was lower than that in the control group( P < 0. 05),and the hospitalization time in the study group was shorter than that in the control group( P < 0. 05). The peripheral white blood cell count( WBC),cytokine interleukin-6( IL-6) and C-reactive protein( CRP) levels in the study group were significantly lower than those in the control group at 24 h and 72 h after surgery( P < 0. 05). Conclusion The application of rapid rehabilitation surgery concept didn’t increase the amount of bleeding during thoracotomy pneumonectomy,which could effectively reduce postoperative complications,reduce various stress response indicators,improve recovery efficiency and effectiveness,shorten hospitalization time,and promote rapid recovery of the body.
