摘要
目的探讨T2DM患者血清神经调节蛋白4(Nrg4)与早期糖尿病肾脏疾病(EDKD)的相关性。方法选取2017年8月至2018年10月于我院肾内科、内分泌科门诊及住院部就诊的40例新诊断单纯T2DM患者(T2DM组)、39例EDKD患者(EDKD组)及同期40名健康体检者(NC组)。ELISA法测定空腹血清Nrg4水平。结果 NC组、T2DM组、EDKD组血清Nrg4水平依次降低(P<0. 05或P<0. 01);血清Nrg4水平与超氧化物歧化酶、HDL-C呈正相关(P<0. 05),与胰岛素抵抗指数(HOMA-IR)、FIns、FPG、HbA1c、单核细胞趋化蛋白1(MCP-1)、UACR、hsC-RP、肿瘤坏死因子α(TNF-α)、病程、BUN、Ucr呈负相关(P<0. 05或P<0. 01)。Logistic回归分析显示,HbA1c、Nrg4是EDKD的影响因素。结论 EDKD患者血清Nrg4水平降低,可能在DKD发生发展中发挥重要作用;Nrg4水平降低可能是EDKD的预测指标。
Objective To explore the association between serum Neuregulin 4(Nrg4)level and early diabetic kidney disease.three groups:newly diagnosed type 2 diabetes(T2 DM group,n=40),early diabetic kidney disease(EDKD group,n=39),and healthy subjects with normal glucose tolerance(NC group,n=40). Fasting serum Nrg4 was measured by ELISA method.group,T2DM group and EDKD group(P<0. 05 or P<0. 01). Nrg4 levels were positively correlated with SOD and HDL-C(P<0. 05),and negatively correlated with HOMA-IR,FIns,FPG,HbA1 c,monocyte chemotactic protein(MCP-1),UACR,hsC-RP,TNF-α,diabetes duration,BUN,and Ucr(P<0. 05 or P<0. 01). Logistic regression analysis showed that Nrg4 and HbA1 c were the independent influencing factors for EDKD.in EDKD. Low Nrg4 maybe a reliable predictor for EDKD.
