鼻内镜手术治疗慢性鼻窦炎伴鼻息肉的临床疗效及对生活质量的影响

Clinical efficacy of nasal endoscopic surgery for treatment of chronic rhinosinusitis accompanied with nasal polyps and its impact on quality of life

目的探讨改良鼻内镜手术对慢性鼻窦炎伴鼻息肉患者的临床症状和对生活质量的影响。方法选取我院2017年11月~2018年10月收治的96例慢性鼻窦炎伴鼻息肉患者作为研究对象,按照就诊顺序随机分为观察组和对照组,各48例。对照组采用传统鼻内镜手术治疗,观察组采用改良鼻内镜手术治疗,分析两组患者SNOT-20评分、SF-36评分评估状况、手术治疗效果和术后并发症的发生率。结果两组患者SNOT-20评分、SF-36评分评估状况,组间比较:术前与术后3个月后比较,差异无统计学意义(P>0.05);组内比较:术前与术后3个月后比较,差异均有统计学意义(P<0.05);观察组的治疗总有效率(95.83%)高于对照组(79.17%),差异有统计学意义(P<0.05);观察组的术后并发症发生率(2.08%)低于对照组(12.50%),差异有统计学意义(P<0.05)。结论鼻内镜手术对慢性鼻窦炎伴鼻息肉患者临床症状和生活质量的改善具有积极意义,改良鼻内镜手术的临床疗效及并发症发生率优于传统鼻内镜手术。

Objective To investigate the impact of modified endoscopic surgery on the clinical symptoms and quality of life of patients with chronic rhinosinusitis accompanied with nasal polyps.Methods Ninety-six with chronic rhinosinusitis accompanied with nasal polyps admitted to our hospital from November 2017 to October 2018 were selected as the study subjects and divided into the observation group and the control group according to the admission order,with 48 patients in each group.The control group was treated with traditional endoscopic surgery and the observation group was treated with modified endoscopic surgery.The SNOT-20 scores,SF-36 scores,surgical treatment effects and incidences of postoperative complications of the two groups were analyzed.Results Regarding to the SNOT-20 scores and SF-36 scores of the two groups,there were no statistically significant between-group differences before surgery and 3 months after surgery(P>0.05),and there were statistically significant intra-group differences between before surgery and 3 months after surgery(P<0.05).The total effective rate of treatment in the observation group(95.83%)was higher than that in the control group(79.17%),with statistically significant difference(P<0.05).The incidence of postoperative complications in the observation group(2.08%)was lower than that in the control group(12.50%),with statistically significant difference(P<0.05).Conclusion Endoscopic surgery has positive effect on the improvement of clinical symptoms and quality of life in patients with chronic rhisinusitis accompanied with nasal polyps.Modified endoscopic surgery has better clinical efficacy and incidence of complications than traditional endoscopic surgery.
