2种点阵激光治疗面部萎缩性痤疮瘢痕疗效观察

摘要 目的:探讨CO2点阵激光和饵点阵激光治疗面部萎缩性痤疮瘢痕的临床效果。方法:选取萎缩性痤疮瘢痕患者42例,随机分为2组,每组各21例,分别行CO2点阵激光和饵点阵激光治疗。每60 d治疗1次,共治疗3次,治疗结束60 d后比较2组痤疮瘢痕权重评分(ECCA)、有效率及不良反应。结果:CO2点阵激光组ECCA评分治疗前为(135.71±23.78)分,治疗后为(60.71±22.93)分,治疗前后比较差异有统计学意义(P<0.05);饵点阵激光组ECCA评分治疗前为(134.76±22.77)分,治疗后为(68.81±24.23)分,治疗前后比较差异有统计学意义(P<0.05);2组间治疗前后比较差异均无统计学意义(P>0.05)。CO2点阵激光组有效率为71.43%,饵点阵激光组为57.14%,2组比较差异无统计学意义(P>0.05)。CO2点阵激光组红肿、疼痛、结痂脱落及色素沉着时间均长于饵点阵激光组。结论:2种点阵激光均有效、安全,CO2点阵激光不良反应多于饵点阵激光。 Objective: To compare the efficacy and adverse reactions of 10 600 nm CO2 lattice laser and 2 940 nm bait: YAG lattice laser in the treatment of facial atrophic acne scar. Methods: 42 patients with atrophic acne scar were randomly divided into two groups, 21 patients in each group were treated with CO2 lattice laser and bait lattice laser respectively.Treatment was given every 60 days, three times in total. The comprehensive clinical efficacy, adverse reactions(local erythema、pain、pigmentation)duration of the two methods after three treatments were compared. Results: The CO2 dot laser group had an ECCA score of(135.71±23.78) before treatment and(60.71±22.93) points after treatment. The difference was statistically significant(P<0.05). The ECCA score of the bait dot laser group was(134.76±22.77) points before treatment and(68.81±24.23) points after treatment. The difference was statistically significant(P<0.05). There was no significant difference between the two groups before and after treatment(P>0.05). The effective rate of CO2 dot laser group was 71.43%, and that of bait dot laser group was 57.14%. There was no significant difference between the two groups(P>0.05). The redness, pain, scarring and pigmentation time of the CO2 dot laser group were longer than those of the bait array laser group. Conclusion: Both lattice lasers are effective and safe. CO2 lattice laser penetration depth is deep, quick effect, but easy to lead to scab pigmentation;bait: YAG lattice laser penetration is weak, need multiple treatment, but can obtain better curative effect and the surrounding tissue thermal damage is small, recovery time is short, pigmentation incidence is low.
