摘要
目的探讨商洛地区食管鳞癌发生情况与人乳头瘤病毒(HPV)感染的相关性.方法采用HPV(23个型)核酸分型检测试剂盒(荧光PCR法)检测商洛地区首次发现的(未行放化疗)食管鳞癌患者食管组织蜡块标本及同期非食管肿瘤(主要是食管黏膜慢性炎症)的石蜡标本中常见的23种HPV基因型感染情况,探讨商洛地区食管鳞癌发生情况与HPV感染的相关性.结果食管鳞癌与非食管肿瘤组织中低危型HPV、高危型HPV感染率比较,差异具有统计学意义(P<0.05).食管鳞癌与非食管肿瘤组织中低危型HPV11亚型、高危型HPV16亚型感染率比较,差异无统计学意义(P>0.05).食管鳞癌组织中高、中、低分化对应的高危型HPV及高危型HPV16亚型感染率比较,差异有统计学意义(P<0.05).在食管鳞癌中,伴随着肿瘤分化程度的降低,高危型HPV及其中HPV16亚型感染率不断升高.结论HPV感染与商洛地区食管鳞癌的发病有关,在食管鳞癌的进展过程中,高危型HPV尤其是HPV16亚型的感染可能是潜在的致病因素.
Objective To investigate the correlation between the incidence of esophageal squamous cell carcinoma and human papillomavirus(HPV) infection in Shangluo. Methods HPV(23 types) nucleic acid typing test kit(fluorescence PCR) was used to detect 23 kinds of HPV genotype infection in the paraffin samples of esophageal tissues of patients with esophageal squamous cell carcinoma(without radiotherapy or chemotherapy) and the paraffin samples of tissues of nonesophageal tumors(mainly chronic inflammation of esophageal mucosa) in the same period in Shangluo, and the correlation between the incidence of esophageal squamous cell carcinoma and HPV infection in Shangluo was discussed. Results There were statistically significant differences in the infection rate of low-risk HPV and high-risk HPV between the tissues of esophageal squamous cell carcinoma and non-esophageal tumors(P <0.05). There were no significant differences in the infection rates of low-risk HPV11 subtype and high-risk HPV16 subtype between the tissues of esophageal squamous cell carcinoma and non-esophageal tumors(P>0.05). There were significant differences in the infection rates of high-risk HPV and high-risk HPV16 subtype in the tissues of esophageal squamous cell carcinoma with high, medium and low differentiation(P <0.05). In esophageal squamous cell carcinoma, the infection rates of high-risk HPV and its HPV16 subtype were increasing with the decrease of tumor differentiation. Conclusion HPV infection is associated with the incidence of esophageal squamous cell carcinoma in Shangluo. In the progression of esophageal squamous cell carcinoma,high-risk HPV, especially HPV16 subtype infection may be a potential pathogenic factor.
