血清总胆红素、糖化血红蛋白及血脂检验在2型糖尿病合并冠心病中的临床意义 被引量:16

摘要 目的 探讨血清总胆红素(TBIL)、糖化血红蛋白(HbA1c)及血脂检验在2型糖尿病合并冠心病中的临床意义.方法 选取本院2016年12月8日至2018年7月22日收诊的2型糖尿病患者与2型糖尿病合并冠心病患者共76例,按照疾病类型不同分为观察组(38例,2型糖尿病合并冠心病)和对照组(38例,2型糖尿病).两组均行TBIL、HbA1c及血脂检验,比较两组的高密度脂蛋白胆固醇(HDL-C)、TBIL、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和HbA1c水平,并分析观察组不同冠脉病变积分(CSS)患者的上述指标水平.结果 观察组的HDL-C、TBIL水平均显著低于对照组,TG、TC、LDL-C、HbA1c水平均显著高于对照组,差异具有统计学意义(P<0.05).随着CSS分值的降低,HDL-C、TBIL水平逐渐升高,差异具有统计学意义(P<0.05);随着CSS分值的降低,TG、TC、LDL-C、HbA1c水平相应降低,差异具有统计学意义(P<0.05).结论 TBIL、HbA1c与血脂检验可对2型糖尿病合并冠心病进行有效诊断,为往后治疗提供准确依据,值得推广使用. Objective To investigate the clinical significance of detection of serum bilirubin(TBIL), glycosylated hemoglobin(HbA1 c) and blood lipids in type 2 diabetes mellitus complicated with coronary heart disease. Methods A total of 76 patients with type 2 diabetes mellitus and type 2 diabetes mellitus complicated with coronary heart disease admitted in our hospital from December 8, 2016 to July 22, 2018 were selected. According to different disease types, the patients were divided into observation group(38 patients, type 2 diabetes mellitus complicated with coronary heart disease) and control group(38 patients, type 2 diabetes mellitus). The detection of TBIL, HbA1 c and blood lipids were performed in both groups, and the levels of high density lipoprotein cholesterol(HDL-C), TBIL, total cholesterol(TC), triacylglycerol(TG), low density lipoprotein cholesterol(LDL-C) and HbA1 c of the two groups were compared. Levels of above indexes in patients with different coronary lesion score(CSS) in the observation group were analyzed. Results The levels of HDL-C and TBIL in the observation group were significantly lower than those in the control group, while the levels of TG, TC,LDL-C and HbA1 c were significantly higher than those in the control group, the differences were statistically significant(P <0.05). With the decrease of CSS score, HDL-C and TBIL levels increased gradually, and the differences were statistically significant(P <0.05). With the decrease of CSS score, TG, TC, LDL-C and HbA1 c levels decreased correspondingly, and the differences were statistically significant(P<0.05). Conclusion The detection of TBIL, HbA1 c and blood lipids can make effective diagnosis for patients with type 2 diabetes mellitus complicated with coronary heart disease,and provide accurate basis for future treatment, which is worth popularizing and using.
