乳腺癌手术中行乳晕下亚甲蓝法前哨淋巴结活检术的效果

Effect of sentinel lymph node biopsy with methylene blue under areola in breast cancer operation

目的 探讨乳腺癌手术中行乳晕下亚甲蓝法前哨淋巴结活检术的效果.方法 选取我院2013年1月至2016年12月收治的100例乳腺癌手术患者作为研究对象,依照随机数字表法划分成研究组(50例,乳腺癌手术中行乳晕下亚甲蓝法前哨淋巴结活检术)与对照组(50例,传统开放式改良根治术).分析研究组的检出情况并比较两组患者的围术期情况、生活质量及并发症发生情况.结果 研究组患者的前哨淋巴结检出灵敏度为86.96%,准确性为84.78%,假阴性率为21.74%,假阳性率为8.70%.研究组患者的手术用时及拔管时间均明显短于对照组;术中失血量及引流量均明显少于对照组(P<0.05).治疗后,两组患者的生活质量各项评分均升高,且研究组显著高于对照组(P<0.05).研究组患者的并发症总发生率显著低于对照组(P<0.05).结论 乳腺癌手术中行乳晕下亚甲蓝法前哨淋巴结活检术可明显缩短手术用时及拔管时间,减少术中失血量及引流量,提高患者生活质量,且术后并发症发生率低.

Objective To investigate the effect of sentinel lymph node biopsy with methylene blue under areola in breast cancer operation. Methods A total of 100 patients with breast cancer surgery admitted in our hospital from January 2013 to December 2016 were selected as the research objects and divided into study group(50 cases, sentinel lymph node biopsy with methylene blue under areola) and control group(50 cases, traditional open modified radical operation) according to the random number table method. The detection in the study group was analyzed and the perioperative situation, quality of life and complications of the two groups were compared. Results In the study group, sentinel lymph node detection sensitivity was 86.96%, accuracy was 84.78%, false negative rate was 21.74% and false positive rate was 8.70%. The operation time and extubation time of the patients in the study group were significantly shorter than those in the control group, and the intraoperative blood loss and drainage volume were significantly less than those in the control group(P <0.05). After treatment, the quality of life scores of the two groups were increased, and the study group were significantly higher than those of the control group(P<0.05). The total incidence of complications in the study group was significantly lower than that in the control group(P <0.05). Conclusion Sentinel lymph node biopsy with methylene blue under areola in breast cancer operation can significantly shorten the operation time and extubation time, reduce the intraoperative blood loss and drainage volume, improve the quality of life of patients, and with lower incidence of postoperative complications.
