膝周射频热凝术联合奇正消痛贴膏治疗膝骨性关节炎疗效及对关节滑液中白介素-1β和肿瘤坏死因子-α水平影响

Effect of radiofrequency thermocoagulation combined with Qi Zheng Xiaotong plaster in treatment of knee osteoarthritis and impact on interleukin-1beta and tumor necrosis factor-alpha in synovia fluid

目的探讨膝周射频热凝术联合奇正消痛贴膏治疗膝骨性关节炎(KOA)对关节滑液中白介素-1β(IL-1β)和肿瘤坏死因子-α(TNF-α)水平影响。方法选择120例KOA患者,观察组A采用膝周射频热凝术治疗,观察组B采用奇正消痛贴膏治疗,观察组C采用膝周射频热凝术联合奇正消痛贴膏治疗;比较治疗前后关节滑液中IL-10和TNF-α浓度变化。结果3组关节滑液中IL-1β和TNF-α比较,治疗前差异无统计学意义(P>0.05);治疗第2周差异有统计学意义(F值分别为7.65、14.50,P<0.01);治疗第4周差异有统计学意义(F值分别为5.05.19.52,P<0.01);3组治疗前后关节滑液中IL-1β和TNF-α比较,差异有统计学意义(F值为4.25~12.51,P<0.01)。结论膝周射频热凝术联合奇正消痛贴膏可显著降低关节滑液中IL-1β和TNF-α水平。

Objective To investigate the impact of radiofrequency thermocoagulation combined with Qi Zheng Xiaotong plaster on the interleukin-1 beta and tumor necrosis factor—alpha in synovia fluid in the treatment of knee osteoarthritis.Methods One hundred and twenty cases of patients with knee osteoarthritis were selected,radiofrequency thermocoagulation was given in the observation group A,Qi Zheng Xiaotong plaster was given in the observation group B,and radiofrequency thermocoagulation combined with Qi Zheng Xiaotong plaster were given to the observation group C.The levels of interleukin-1 beta and tumor necrosis factor-αlpha concentration in the synovial fluid were detected before and after treatment.Results The levels of interleukin-1 beta and TNF-alpha in synovial fluid were compared,there was no statistical significance on the difler-ence before treatment(P>0.05);there was statistical significance on the difference on the second week of treatment(F=7.65,F=14.50,P<0.01)and on the fourth week of treatment(F=5.05,F=19.52,P<0.01).The concentrations of interleukin 1 beta and tumor necrosis factor-αlpha in synovial fluid before and after treatment among the 3 groups were compared,and there was statistical significance on the differences(F=4.25-12.51,P<0.01).Conclusion Radiofrequency thermocoagulation combined with Qi Zheng Xiaotong plaster can significantly reduce the levels of interleukin-1 beta and tumor necrosis factor-αlpha in synovial fluid.
