单硝酸异山梨酯缓释片、多巴胺、呋塞米联合治疗慢性肺源性心脏病心力衰竭临床价值体会

Clinical value of combined therapy of isosorbide mononitrate sustained-release tablets, dopamine and furosemide in the treatment of chronic pulmonary heart disease and heart failure

目的探讨单硝酸异山梨酯缓释片、多巴胺、呋塞米联合治疗慢性肺源性心脏病心力衰竭临床价值。方法80例慢性肺源性心脏病心力衰竭患者,按照随机抽签法分为研究组和对照组,每组40例。对照组给予单硝酸异山梨酯缓释片治疗,研究组给予单硝酸异山梨酯缓释片、多巴胺、呋塞米联合治疗。观察比较两组的临床效果、治疗前后B型脑钠肽(BNP)水平、治疗后6 min步行距离。结果研究组患者治疗总有效率97.50%高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗后,研究组和对照组患者BNP水平分别为(134.65±37.80)、(335.75±87.50)pg/ml,均低于治疗前的(541.50±102.35)、(541.45±102.40)pg/ml,研究组BNP水平低于对照组,差异具有统计学意义(P<0.05)。治疗后,研究组患者6 min步行距离(389.20±49.50)m长于对照组的(358.65±56.70)m,差异有统计学意义(P<0.05)。结论慢性肺源性心脏病心力衰竭应用单硝酸异山梨酯缓释片、多巴胺、呋塞米联合治疗可以有效提高临床疗效,改善临床症状,提高生活质量,值得推广应用。

Objective To discuss the clinical value of combined therapy of isosorbide mononitrate sustained-release tablets,dopamine and furosemide in the treatment of chronic pulmonary heart disease and heart failure.Methods A total of 80 patients with chronic pulmonary heart disease and heart failure were divided into research group and control group by lottery method,with 40 cases in each group.The control group was treated with isosorbide mononitrate sustained-release tablets,and the research group was treated with combined therapy of isosorbide mononitrate sustained-release tablets,dopamine and furosemide.The clinical effect,B-type natriuretic peptide(BNP)level before and after treatment,and 6-min walking distance after treatment were observed and compared between the two groups.Results The total effective rate of treatment 97.50%in the research group was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,the BNP levels were(134.65±37.80)and(335.75±87.50)pg/ml respectively in the the research group and control group,which were lower than(541.50±102.35)and(541.45±102.40)pg/ml before treatment.The BNP level in the research group was lower than that in the control group.Their difference was statistically significant(P<0.05).After treatment,the 6-min walking distance(389.20±49.50)m in the research group was longer than(358.65±56.70)m in the control group,and the difference was statistically significant(P<0.05).Conclusion Combined therapy of isosorbide mononitrate sustained-release tablets,dopamine and furosemide can effectively improve clinical efficacy,clinical symptoms and quality of life of patients with chronic pulmonary heart disease and heart failure.
