耳穴贴压联合抗高眩方治疗原发性高血压病眩晕的临床研究

Clinical Study on Auricular Acupoint Sticking Combined with Kanggaoxuan Recipe in Treating Vertigo in Essential Hypertension

[目的]观察耳穴贴压联合抗高眩方治疗原发性高血压病眩晕的临床疗效.[方法]将60例原发性高血压病眩晕患者随机分为治疗组和对照组,每组各30例.对照组给予苯磺酸氨氯地平分散片,阿司匹林肠溶片降压治疗,治疗组在对照组用药基础上,加予耳穴贴压与抗高眩方治疗,均连续治疗4周.比较2组患者的临床疗效,治疗前后眩晕程度评分、中医症状总积分和血压,以及血浆血栓前状态(PTS)分子标志物(vWF、DD、PA-1、FIB)的情况.[结果](1)治疗组总有效率为90.00%,对照组为80.00%.治疗组疗效优于对照组,差异有统计学意义(P<0.05).(2)治疗后,2组患者眩晕程度评分、中医症状总积分和血压均有明显改善(P<0.05或P<0.01),且治疗组的改善作用明显优于对照组,差异均有统计学意义(P<0.05或P<0.01).(3)治疗后,2组患者血浆vWF,DD,PA-1,FIB均有明显改善(P<0.05或P<0.01),且治疗组的改善作用优于对照组(P<0.01).[结论]耳穴贴压联合抗高眩方治疗原发性高血压病眩晕的疗效显著,能明显地降低血压,改善患者的临床症状,可能与改善患者血栓前状态有关.

Objective To observe the clinical effect of auricular acupoint sticking combined with Kanggaoxuan Recipe in treating vertigo in essential hypertension.Methods Sixty patients with essential hypertension-induced vertigo were randomly divided into treatment group and control group,30 cases in each group.The control group received oral use of Amlodipine Besylate Dispersible Tablets and Asipilin Enteric-coated Tablets,and the treatment group received auricular acupoint sticking and Kanggaoxuan Recipe based on the treatment for the control group,the treatment lasting 4 continuous weeks.After treatment,we compared the clinical efficacy,and the vertigo degree scores,total scores of traditional Chinese medicine symptoms,blood pressure and plasma molecular markers of prethrombotic state such as von Willebrand factor(vWF),D-dimer(DD),plasminogen activator inhibitor 1(PAI-1)and fibrinogen(FIB)in the two groups before and after treatment.Results(1)The total effective rate was 90.00%in the treatment group and was 80.00%in the control group,and the curative efficacy of the treatment group was superior to that of the control group,the difference being significant(P<0.05).(2)After treatment,the scores of the vertigo degree and traditional Chinese medicine symptoms,and blood pressure were improved in the two groups(P<0.05 or P<0.01),and the improvement in the treatment group was superior to that in the control group,the difference being significant(P<0.05 or P<0.01).(3)After treatment,the levels of plasma vWF,DD,PAI-1 and FIB in the two groups were decreased(P<0.05 or P<0.01),and the improvement in the treatment group was superior to that in the control group(P<0.01).Conclusion Auricular acupoint sticking combined with Kanggaoxuan Recipe is effective for the treatment of vertigo in essential hypertension,and can significantly decrease the blood pressure and improve clinical symptoms in the patients,and its mechanism may be related to the improvement of pre-thrombotic state.
