Objective To summarize the clinical experience and accuracy of the detection algorithm on atrial fibrillation(AF)by insertable cardiac monitor(ICM)Reveal LINQ in patients with cryptogenic stroke.Methods This is a single-center,observational study of consecutive patients with cryptogenic stroke undergoing ICM Reveal LINQ implantation from September 11,2014 to September 10,2017.Inclusion criteria were as follows:acute ischemic stroke or transient ischemic attack(TIA)within 6 months confirmed by head MRI;cryptogenic stroke identified by a neurological specialist;no previous AF identified by 12-lead ECG,cardiac monitoring and at least 24-hour Holter.Results A total of 23 consecutive patients were enrolled in this study with mean age of(68±9)years(46-83 years).Implantation procedure was successful in all patients without complication.During the median follow-up of 369 days(17-944 days),no battery failure occurred,and ICM was removed in 4 patients.The accuracy of AF detection was 21.2%(62/293).During the follow-up,AF was detected in 21.7%of patients(5/23).The cumulative detection rate of AF during the 1,1.5,2 and 2.5 years follow up was 11.2%,19.2%,35.4%and 56.2%.Conclusion The clinical use of ICM Reveal LINQ is safe with a necessary of improving the detection algorithm for AF.In cryptogenic stroke patients without previous AF post the use of cardiac monitor and Holter et al,the AF detection rate by ICM was as high as 21.7%.However,the clinical characteristics,prognosis and treatment of AF detected by implantable device need to be elucidated by future studies.
