摘要
目的评估磁共振T1WI信号强度比(signal intensity ratio on T1WI,SIR1)对多发性骨髓瘤(MM)骨盆病变治疗效果的评价价值.方法收集本院2017年12月至2019年5月间收治的多发性骨髓瘤患者25例;另选取健康志愿者10例作为正常对照组.采用1.5T磁共振扫描仪对MM患者治疗前后进行骨盆MRI成像检查,结合骨髓穿刺结果,观察治疗前后MM患者骨盆磁共振信号变化,测量分析相应感兴趣区的T1WI信号噪声比(SIR1)的差异.结果25例MM患者经规范化疗后,14例接近完全缓解,11例患者未缓解.14例治疗后缓解者的骨髓信号T1WI上信号增高,T1WI骨盆骨髓SIR1值为(2.967±0.342),与正常对照组(3.126±0.342)比较,差异无统计学意义(P>0.05);11例化疗后未缓解者T1WI骨盆骨髓SIR1值为(1.896±0.521),与正常对照组(3.126±0.342)比较,差异有统计学意义(P<0.05).结论MR-SIR1可作为一种简便有效的定量评价MM疗效的无创性影像学检测手段.
Objective To assess the value of signal intensity ration on T1W1(SIR1)in the evaluation of the treatment effects of multiple myeloma(MM)pelvic lesions.Methods 35 patients with MM were enrolled into this study and received magnetic resonance imaging(MRI)scanning before and after treatment.The differences of SIR1 in regions of interest were analyzed by MRI scanning combined with the results of bone marrow puncture.Results The 35 cases were treated with standardized therapy;and 14 got near complete remission,and 11 no remission.The signal of bone marrow on T1WI increased in the 14 patients with remission after treatment,and the SIR 1 value of pelvic bone marrow on T1WI was(2.967±0.342),with no statisitcal difference from that of the normal control group[(3.126±0.342)](P>0.05).The SIR 1 value of pelvic bone marrow on T1WI in the 11 patients without remission after chemotherapy was(1.896±0.521),which was statistically different from that of the control group[(3.126±0.342)](P<0.05).Conclusion MR-SIR1 can be used as a simple and noninvasive imaging detection method for assessing the treatment effects of MM.
