10例儿童心脏肿瘤病例分析

Case analysis of 10 children with cardiac tumors

目的总结小儿心脏肿瘤的治疗效果.方法收集2013年10月1日至2018年10月1日由本院确诊并行手术治疗的心脏肿瘤患儿10例作为研究对象,回顾性分析患儿的年龄、临床表现、肿瘤部位、诊断、治疗、病理分型及预后等情况.结果10例患儿中,男9例,女1例,年龄范围1.3月~9岁,年龄中位数2岁4月;临床表现多以胸闷、气促为主;患儿均行心脏彩超检查,7例心脏肿瘤完整切除,3例心脏肿瘤部分切除;横纹肌瘤5例,纤维性肿瘤1例,纤维母细胞瘤1例,脂肪瘤1例,转移性肿瘤2例.1例死亡,8例随访5月~5年,未出现肿瘤复发.结论儿童心脏肿瘤症状多不典型,多在婴幼儿期间诊断,对有明显症状者应及时采取手术治疗,手术以恢复正常血流动力学状态为目的.预后与肿瘤病理结果相关.

Objective To summarize the therapeutic effect of pediatric cardiac tumors.Methods 10 children with cardiac tumors diagnosed and surgically treated in our hospital from October 1,2013 to October 1,2018 were collected.The age,clinical manifestations,tumor location,diagnosis,treatment,pathological classification,and prognosis were retrospectively analyzed.Results Among the 10 children,9 were boys,and 1 girl.The age ranged from 1.3 months to 9 years,with a median age of 2 years and 4 months.Most of the clinical features were chest tightness and shortness of breath.All the children underwent color Doppler echocardiography.7 cases had complete resection of cardiac tumors,and 3 partial resection.There were 5 cases of rhabdomyoma,1 case of fibrous tumor,1 case of fibroblastoma,1 case of lipoma,and 2 cases of metastatic tumors.One case died.Eight cases were followed up from 5 months to 5 years,and no recurrence was found.Conclusions The symptoms of cardiac tumors in children are atypical,and most of them are diagnosed in infants and young children.Surgical operation should be taken in time for those with obvious symptoms.The operation purpose is to restore normal hemodynamic state.The prognosis is correlated with the pathological results of the tumors.
