香砂六君子汤联合肾康注射液治疗慢性肾衰竭疗效观察

Therapeutic effect observation of Xiangsha Liujunzi Decoction combined with Shenkang Injection on chronic renal failure

目的:观察香砂六君子汤中药煎剂联合肾康注射液治疗慢性肾衰竭(CRF)的临床疗效.方法:选择2012年9月-2014年9月连云港市中医院的住院患者49例,均符合CRF诊断标准,男33例,女16例;年龄35~80岁,平均年龄(62.29±12.13)岁.49例CRF患者在采取基础治疗(予以低盐低脂优质低蛋白饮食,治疗上包括控制血压、控制血糖、纠正电解质紊乱及酸碱平衡失调、降甲状旁腺激素、改善贫血、减轻胃肠道症状、抗感染等对症处理)的同时予以香砂六君子汤中药煎剂口服联合肾康注射液静滴,观察治疗前后患者肾功能的变化情况,即尿素氮(BUN)、肌酐(SCr)、β2-微球蛋白(β2-M)、肾小球滤过率(eGFR)的变化情况.结果:治疗后,49例CRF患者中显效10例,有效12例,稳定20例,无效7例,总有效率为85.7%;BUN、SCr、β2-M、eGFR均有明显改善,与治疗前比较,差异有统计学意义(P<0.05).结论:香砂六君子汤联合肾康注射液能有效改善早中期慢性肾衰竭患者的肾功能水平.治疗后患者的肾功能有所改善,并能改善患者的临床症状,提高患者生活质量,尽量使患者血液透析的时间延后.因此,肾康注射液静滴可协同中药香砂六君子汤通过不同给药途径,共奏补中益气、泄浊利湿、益气活血之功.

Objective:To observe the clinical effect of Xiangsha Liujunzi Decoction(香砂六君子汤)combined with Shenkang injection in the treatment of chronic renal failure(CRF).Methods:From September 2012 to September 2014,49 patients admitted to Lianyungang Hospital of Traditional Chinese Medicine were eligible for CRF.There were 33 males and 16 females,aged 35 to 80 years,with an average age of 62.29±12.13 years.49 patients with CRF were treated with basic therapy(low-salt,low-fat,high-quality,low-protein diet,including blood pressure control,blood sugar control,electrolyte imbalance,acid-base balance,hypoparathyroidism,anemia,and reduce gastrointestinal symptoms,anti-infective and other symptomatic treatments).At the same time,Xiangsha Liujunzi Decoction Chinese herbal decoction was combined with Shenkang injection to observe the changes of renal function before and after treatment,that was,changes of urea nitrogen(BUN),creatinine(SCr),β2-microglobulin(β2-M),and glomerular filtration rate(eGFR).Results:After treatment,out of 49 cases,it was markedly e ffective in 10 cases,e ffective in 12 cases,stable in 20 cases,invalid in 7 cases,the total effective rate was 85.7%.Urea nitrogen,creatinine,β2-M,eGFR were significantly improved compared with before treatment,the difference was statistically significant(P<0.05).Conclusion:Xiangsha Liujunzi Decoction combined with Shenkang injection could effectively improve the renal function of patients with early and middle stage of CRF.After treatment,the patient’s renal function was improved,and the clinical symptoms of the patient could be relieved,the quality of life of the patient was improved,and the time for hemodialysis of the patient could be delayed as much as possible.Therefore,Shenkang Injection could be combined with traditional Chinese medicine Xiangsha Liujunzi Decoction through different routes of administration to play the role of dispelling turbidity and dampness,benefiting Qi and promoting blood circulation.
