中医辨证护理在增殖性糖尿病视网膜病变出血术后的应用

Application of TCM syndrome differentiation nursing in postoperative period of proliferative diabetic retinopathy hemorrhage

目的探讨增殖性糖尿病视网膜病变出血患者采用中医辨证护理对其术后视力及生活质量的影响。方法选取2017年3月至2019年3月118例在本院行手术治疗的增殖性糖尿病视网膜病变出血患者,根据随机数字表法进行分组,各59例。对照组采用常规护理措施,观察组在对照组基础上采用中医辨证护理,对比两组视力水平、糖尿病生活质量及并发症。结果护理前,两组患者的视力水平比较差异无统计学意义(P>0.05)。护理后两组视力水平均提高,观察组更高,两组比较差异有统计学意义(P<0.05);两组治疗维度、社会关系维度比较差异无统计学意义(P>0.05);两组心理/精神维度、生理维度评分降低,观察组更低,两组比较差异有统计学意义(P<0.05)。观察组患者术后并发症发生率低于对照组[6.78%(4/59)比22.03%(13/59)],两组比较差异有统计学意义(P<0.05)。结论中医辨证护理可有效改善增殖性糖尿病视网膜病变出血患者术后视力水平,控制糖代谢指标,预防眼压波动,进而提高生活质量。

Objective To investigate the effect of TCM syndrome differentiation nursing on postoperative visual acuity and quality of life in patients with proliferative diabetic retinopathy hemorrhage.Methods 118 cases of patients with proliferative diabetic retinopathy hemorrhage who underwent surgery in our hospital from March 2017 to March 2019 were enrolled in this study.All patients were divided into two groups according to the random number table method,with 59 cases in each group.The patients in the control group were treated with routine nursing interventions,and the patients in the observation group were treated with TCM syndrome differentiation nursing intervention.The visual acuity level,diabetes quality of life,and complications were compared between the two groups.Results Before nursing,there was no statistically significant difference in visual acuity between the two groups(P>0.05);after nursing,the visual acuity of the two groups improved,and the visual acuity of the observation group was higher than that of the control group(P<0.05).There were no statistically significant differences in the dimensions of treatment and social relationship between the two groups(P>0.05);after nursing,the scores of psychological/spiritual dimension and physiological dimension of the two groups decreased,and those of the observation group were even lower(P<0.05).The incidence of postoperative complications in the observation group was 6.78%(4/59),which was lower than that in the control group[22.03%(13/59)](P<0.05).Conclusion TCM syndrome differentiation nursing can effectively improve postoperative visual acuity in patients with proliferative diabetic retinopathy hemorrhage,control glucose metabolism,prevent intraocular pressure fluctuations,and improve quality of life.
