不同拔尿管方法在泌尿外科老年手术患者护理中的应用

Application of different removal methods of urinary catheter in nursing of elderly patients undergoing urology surgery

目的分析不同拔尿管方法在泌尿外科老年手术患者护理中的应用价值。方法选取2018年3月至2019年3月期间本院泌尿外科收治的老年手术患者80例,按拔尿管方法不同分为研究组(n=40)与参照组(n=40),两组均行对症手术治疗,术后留置双腔气囊导尿管,参照组予以传统拔尿管法,研究组予以改良拔尿管法。对比两组拔除尿管时舒适情况、拔除尿管后排尿情况、并发症。结果研究组无特殊不适率较参照组高[82.50%(33/40)比17.50%(7/40)],稍有疼痛率较参照组低[17.50%(7/40)比60.00%(24/40)],明显疼痛较参照组低[0.00%(0/40)比22.50%(9/40)],两组比较差异均有统计学意义(均P<0.05);研究组自行排尿率较参照组高[97.50%(39/40)比77.50%(31/40)],研究组诱导排尿率较参照组低[2.50%(1/40)比22.50%(9/40)],两组比较差异均有统计学意义(均P<0.05);研究组尿路刺激症发生率较参照组低[7.50%(3/40)比35.00%(14/40)],尿潴留发生率较参照组低[0.00%(0/40)比12.50%(5/40)],两组比较差异均有统计学意义(均P<0.05)。结论泌尿外科老年手术患者采用改良拔尿管法可提高拔管时舒适度,促使患者自行排尿,降低并发症发生率,值得临床推广应用。

Objective To analyze the application value of different removal methods of urinary catheter in the nursing of elderly patients undergoing urology surgery.Methods Eighty elderly patients undergoing urology surgery in our hospital were selected from March 2018 to March 2019.The patients were divided into study group(n=40)and reference group(n=40)according to the removal methods of urinary catheter.Symptomatic surgery was performed,and a double-chamber balloon catheter was placed after operation.The reference group was given conventional removal method of urinary catheter,and the study group was treated with modified removal method of urinary catheter.The comfort of the two groups when removing urinary catheter,the urination after removal of urinary catheter,and complications were compared.Results The rate of no special discomfort in the study group was higher than that in the reference group[82.50%(33/40)vs.17.50%(7/40)],the rate of slight pain was lower than that in the reference group[17.50%(7/40)vs.60.00%(24/40)],the rate of obvious pain was lower than that in the reference group[0.00%(0/40)vs.22.50%(9/40)](P<0.05).The rate of voluntary urination in the study group was higher than that in the reference group[97.50%(39/40)vs.77.50%(31/40)],the rate of induced urination was lower than that in the reference group[2.50%(1/40)vs.22.50%(9/40)](P<0.05).The incidences of urinary tract irritation,urinary retention in the study group were lower than those in the reference group[7.50%(3/40)vs.35.00%(14/40),0.00%(0/40)vs.12.50%(5/40)](P<0.05).Conclusion The modified removal method of urinary catheter in urological elderly patients can improve the comfort of extubation,prompt patients to urinate by themselves,and reduce the incidence of complications,which is worthy of clinical application.
