厄贝沙坦联合阿魏酸钠对糖尿病肾病患者肾间质纤维化及凝血功能的影响

Effect of irbesartan and sodium ferulate on renal interstitial fibrosis and coagulation function in patients with diabetic nephropathy

目的探讨厄贝沙坦联合阿魏酸钠对糖尿病肾病(DN)患者肾间质纤维化及凝血功能的影响。方法选取2015年2月至2016年6月本院治疗的DN患者96例,随机数字表法分为两组,各48例。对照组在常规方案上联合厄贝沙坦方案治疗,观察组在对照组基础上联合阿魏酸钠方案治疗。比较两组患者治疗前后肾间质纤维化水平及凝血功能指标。结果治疗3个月后,观察组血清CTGF、血清AngⅡ、尿CTGF、尿AngⅡ水平分别为(103.12±9.03)ng/L、(72.13±8.95)ng/L、(40.24±5.64)ng/mg、(92.13±8.95)ng/mg,均低于对照组,差异均有统计学意义(均P<0.05);观察组凝血酶时间(TT)、活化部分凝血活酶时间(APTT)指标分别为(18.12±1.43)s、(32.13±2.95)s,均高于对照组,纤维蛋白原(FIB)水平为(1.84±0.64)g/L,低于对照组,差异均有统计学意义(均P<0.05)。结论厄贝沙坦联合阿魏酸钠能改善DN患者肾间质纤维化水平,提升凝血功能阻止病情恶化。

Objective To investigate the effects of irbesartan combined with sodium ferulate on renal interstitial fibrosis and coagulation function in patients with diabetic nephropathy(DN).Methods Ninetysix patients with DN who were admitted to our hospital from February 2015 to June 2016 were enrolled,and were divided into control group and observation group with the random number table method,with 48 cases in each group.The two groups were treated with conventional treatment.The irbesartan regimen was administered to the control group on the basis of conventional program,and the sodium ferulate was administered to the observation group on the basis of the control group.The renal interstitial fibrosis level and coagulation functional indicators were compared before and after treatment.Results After 3 months of treatment,the levels of serum CTGF,serum AngII,urinary CTGF,and urinary AngII in the observation group were(103.12±9.03)ng/L,(72.13±8.95)ng/L,(40.24±5.64)ng/mg,(92.13±8.95)ng/mg respectively,which were lower than those in the control group,with statistically significant differences(P<0.05).The thrombin time(TT)and activated partial thromboplastin time(APTT)in the observation group were(18.12±1.43)s,(32.13±2.95)s,which were higher than those in the control group,and the fibrinogen(FIB)level was(1.84±0.64)g/L,which was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion Irbesartan combined with sodium ferulate can improve the level of renal interstitial fibrosis in DN patients and improve coagulation function to prevent the disease from worsening.
