枸橼酸咖啡因对早产儿原发性呼吸暂停及相关并发症的影响

摘要 目的分析枸橼酸咖啡因对早产儿原发性呼吸暂停及相关并发症的影响。方法选择2018年4月~2019年3月我院收治的早产儿80例,采用随机数字表法分为对照组和观察组,各40例。对照组采用氨茶碱治疗,观察组采用枸橼酸咖啡因治疗,比较两组临床治疗总有效率、每天呼吸暂停发生频率、用氧时间、持续气道正压通气(NCPAP)时间、并发症发生情况以及临床不良反应。结果观察组治疗总有效率为92.50%,高于对照组的80.00%,差异有统计学意义(P<0.05);观察组每天呼吸暂停发生频率、用氧时间、NCPAP时间均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为5.00%,低于对照组的17.50%,差异有统计学意义(P<0.05)。结论枸橼酸咖啡因对早产儿原发性呼吸暂停治疗具有积极的促进作用,减少呼吸暂停发生,可一定程度缩短用氧和NCPAP时间,预防并发症的发生,具有一定的临床应用价值。 Objective To analyze the effect of caffeine citrate on primary apnea and related complications in premature infants.Methods 80 premature infants admitted to our hospital from April 2018 to March 2019 were randomly divided into the control group and the observation group,40 cases in each group.The control group was treated with aminophylline,and the observation group was treated with caffeine citrate.The total effective rate of clinical treatment,the frequency of daily apnea,oxygen time,continuous positive airway pressure(NCPAP)time,and complications were compared.Occurrence and clinical adverse reactions.Results The total effective rate of the observation group was 92.50%,which was higher than that of the control group 80.00%,the difference was statistically significant(P<0.05).The frequency of daily apnea,oxygen time and NCPAP time in the observation group were lower than the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.00%,which was lower than 17.50%in the control group,the difference was statistically significant(P<0.05).Conclusion Caffeine citrate has a positive effect on the treatment of primary apnea in premature infants.It can reduce the occurrence of apnea and shorten the time of oxygen and NCPAP to prevent complications,it has certain clinical application value.
